Abstract [sv]

Bakgrund: Palliativ vård innebär vård som ges till patienten den sista tiden i livet och syftar till en förbättrad livskvalitet. Inom sjuksköterskans ansvarsområde ingår att främja hälsa och lindra lidande, vilket beskrivs i sjuksköterskans etiska kod. Sjuksköterskans roll innefattar att stötta, skapa goda relationer och tillgodose en god kommunikation. Palliativ vård utgår från de fyra hörnstenarna, vilka är symtomlindring, teamarbete, kommunikation och relation samt närståendestöd.

Syfte: Att belysa sjuksköterskans erfarenheter av att vårda patienter i behov av palliativ vård inom slutenvården.

Metod: Litteraturöversikten baseras på 15 vetenskapliga studier.

Resultat: Fyra huvudkategorier framkom: Kommunikationens betydelse, Vårdrelationers betydelse, Organisationens betydelse och Sjuksköterskans engagemang. Utifrån dessa kategorier framkom även subkategorier: Kommunikationshinder, Beslutstagande och övergång till palliativ vård, Närståendes betydelse, Stöd till patienten, Psykisk påverkan, Fysisk påverkan och Arbetstillfredsställelse. Under dessa kategorier beskrivs sjuksköterskans erfarenheter av att vårda patienter i behov av palliativ vård inom slutenvården samt faktorer som påverkar sjuksköterskans erfarenheter.

Slutsats: Sjuksköterskans erfarenheter var relaterade till kommunikation, vårdrelationer, organisationen och engagemang. Sjuksköterskan hade erfarenheter av att kommunikationshinder och beslutstaganden påverkade den palliativa vården, vilket äventyrade patientens vård. Sjuksköterskan hade även erfarenheter av att goda vårdrelationer med närstående var gynnsamt för patientens vård. Brist på tid, stöd och resurser samt behov av kompetens, kunskap och utbildning var betydelsefulla faktorer, vilka hade betydelse för vårdkvaliteten.

Abstract [en]

Background: Palliative care means care that is given to the patient in the end of life and aims to improve the patients quality of life. The nurse’s responsibilities includes promoting health and relieve suffering, which is described in the nurse’s code of ethics. The nurse’s role includes supporting, creating good relationships and providing good communication. Palliative care is based on the four cornerstones, which are symptom relief, teamwork, communication and relationship together with support to close relatives.

Aim: To illuminate nurse’s experience of caring for inpatients in need of palliative care.

Method: This literature review is based on 15 scientific studies.

Results: Four main categories emerged: The significance of communication, The significance of care relations, The significance of organization and Nurse’s commitment. Subcategories emerged from these categories: Communication barriers, Decision-making and transition to palliative care, Significance of close relatives, Support to the patient, Mental impact, Physical impact and Job Satisfaction. These categories describe nurse’s experience of caring for inpatients in need of palliative care as well as factors affecting nurse’s experience.

Conclusion: The nurse’s experience were related to communication, care relations, the organization and commitment. The nurse had experienced that communication barriers and decisions affected palliative care, which jeopardized the patient’s care. The nurse had also experienced that good care relationships with close relatives were favorable to the patient care. Lack of time, support and resources as well as the need for skills, knowledge and education were factors that had significance, which were important for the quality of care.
