Abstract [sv]

Syfte: Att belysa distriktssköterskors upplevelser av arbetet med identifiering av depression hos vuxna och äldre inom primärvården.

Metod: Data samlades in från nio intervjuer och analyserades genom kvalitativ innehållsanalys.

Resultat: Utifrån analysen presenterades följande fyra kategorier: "Generella upplevelser", "Svårigheter vid identifiering", "Faktorer som underlättar" och "Nödvändiga förbättringsåtgärder". De flesta av studiens deltagare arbetade mer eller mindre med identifiering av depression. De upplevde att identifieringen till största del gjordes i telefon men att själva diagnostiseringen var en läkaruppgift. Depression upplevdes som ett vanligt förekommande besvär inom primärvården och patienter i alla åldrar drabbades. Deltagarna upplevde att tidsbrist, brist på personalresurser och otillräcklig kunskap försvårade arbetet.

Slutsats: Distriktssköterskorna upplevde arbetet med depression som både intressant och utmanande. Identifieringen skedde till stor del vid telefonrådgivning eller när patienterna kom till vårdcentralen/hälsocentralen av andra anledningar. Det kunde även ske vid utredning av somatiska symtom. Suicidriskeliminering upplevdes som en av de större delarna i arbetet. Distriktssköterskorna efterfrågade ytterligare kunskap, mer tid och bättre resurser som kunde underlätta. Det fanns också en önskan om ytterligare kompetens i form av en psykiatrisjuksköterska på varje vårdcentral. Författarna anser att ytterligare forskning kan vara av vikt för framtida förbättringsåtgärder.

Abstract [en]

Objective: To describe district nurses' experiences of working with identification of depression in adults and older people in primary care.

Method: Data were collected from nine interviews and analysed by qualitative content analysis.

Results: The analysis presented four categories: "General experiences," Difficulties in the identification" "Factors that facilitate" and "Necessary improvements". Most of the participants in the study worked more or less with identification of depression. They felt that the identification mostly took place on the phone and that the actual diagnosis was a medical mission. Depression was perceived as a common disorder in primary care and patients of all ages were affected. Participants felt that the lack of time, lack of staff resources and inadequate knowledge hampered the work.

Conclusion: The district nurses experienced the work with depression as both interesting and challenging. The identification was done mostly by telephone, or when patients came to the health centre for other reasons. It could also be done in the investigation of somatic symptoms. Suicide elimination was perceived as one of the major parts of the work. The district nurses demanded facilitating factors as more knowledge, more time and more resources. There was a desire for additional skills as a psychiatric nurse in each health centre. The authors consider that additional research may be of importance for further improvement
