Bakgrund

Allt fler svårt sjuka äldre vårdas i hemmen vilket ställer krav på en högre kompetens hos både sjuksköterskor och läkare. Dessa två professioner har olika ansvarsområden som behöver kunna samarbeta för att vården ska bli patientsäker. Syftet med studien var att beskriva sjuksköterskans erfarenheter av samarbetet med läkare i hemsjukvården ur ett patientsäkerhetsperspektiv. Metod Studien är kvalitativ med en induktiv ansats som baseras på åtta intervjuer med sjuksköterskor som arbetar inom hemsjukvården. En kvalitativ innehållsanalys har använts vid analysering av material. I resultat framkom tre kategorier: Brister i organisationen kan ge problem för patientsäkerheten, Kommunikation har stor betydelse för patientsäkerheten och Brist på professionellt förhållningssätt försämrar samarbete, ur dessa tre kategorierna skapades underkategorier. Resultatet beskrivs i löpande text som förtydligas med citat. Slutsats För att förbättra samarbete mellan läkaren och sjuksköterskan finns det behov av förändringar i organisationen. Läkaren bör få mer resurser för hemsjukvårdspatienter och vara mer tillgängliga för att kunna stödja sjuksköterska i medicinska beslut. Läkaren behöver ha större kunskap om vad hemsjukvård innebär och sjuksköterskans yrkesansvar En större förståelse för varandras profession och en mer jämlik syn skulle förmodligen resultera i ett bättre samarbete och i en mer patientsäker vård.

Abstract [en]

Background

: More and more elderly people, who need medical healthcare, are nursed in their homes, which require higher demands of competence for both doctors and nurses. The two professions have different areas of responsibilities, therefor the ability to co-operate is crucial for a safer health care for the patients. The purpose of this study is to describe the nurses’ experiences of working together with doctors in home health care from the view of the patients’ safety. Method The study is qualitative with an inductive approach, which is based on eight interviews with nurses who work in home health care. A qualitative analyses has been used for analyzing the material. Results Three categories became clear. Deficiencies in organization may produce problems with patients´ safety, Communication has a significant meaning for the safety of the patients and finally, A lack of professional competence reduces the opportunity of a successful co-operation. From these three main categories sub categories were formed. The result is described in prose, and clarified with quotes. Conclusion In this study the nurses in home health care describe, from their point of view, that the co-operation between the nurses and the doctors needs to improve to preserve a good standard regarding the patients’ safety. To improve this co-operation there is a need to make changes in the organization of home health care. The doctors ought to receive more resources for homecare patients and be more accessible to support the nurses in their medical decisions. Furthermore, the doctors need to have a wider understanding of the specific typical demands and needs that is required in home health care and be aware of the limits of the nurses´ responsibility. A wider understanding of each other´s professions and a more equal status would probably result in a better co-operation between doctors and nurses and a safer care for the patients.
