Abstract [sv]

Bakgrund: Majoriteten av personer som dör i Sverige är i behov av palliativ vård. Hälso- och sjukvården skall erbjuda kvalificerad palliativ vård i linje med WHO´s fyra grundläggande hörnstenar som innefattar symtomlindring, teamarbete, kommunikation och stöd till närstående. Utifrån dessa skall vården utformas individuellt anpassat utefter patientens önskemål och behov. Sjuksköterskor med sitt omvårdnadsansvar är de som till största del möter patienterna på alla vårdavdelningar på sjukhus och har en central roll i den palliativa vården.Syfte: Att beskriva sjuksköterskors erfarenheter av palliativ vård av vuxna patienter inom slutenvården.Metod: Examensarbetet utfördes som en litteraturöversikt där 18 vetenskapliga artiklar inkluderades, både med kvalitativ- och kvantitativ ansats. Artiklarna erhölls från databaserna CINAHL och PubMed samt var publicerade mellan åren 2012 – 2021.Resultat: Bristande kommunikation i vårdteamet med patient och dennes närstående var tydligt framträdande som ett hinder för fullgod palliativ vård. Bristande kunskap framkallade känslor som osäkerhet och otillräcklighet i mötet med personen i livets slut. Sjuksköterskorna hade erfarenheter som att kombinationen av kurativt- och palliativt vårdande utgjorde en tids- och platsbrist, vilket hade en negativ inverkan på vårdkvaliteten.Slutsats: Sjuksköterskor inom slutenvården vittnar om bristande möjligheter till att utföra palliativ vård med god kvalitet i enighet med de grundläggande palliativa hörnstenarna. Brist på tid, kunskap och kommunikation ses vara hinder i utförandet av palliativ vård, vilket gör att patienter i livets slutskede inte får möjlighet till ett personcentrerat slut. Sammantaget visar resultatet att i nuläget är slutenvården i allmänhet ingen bra plats att vårdas palliativt på.

Abstract [en]

Background: The majority of people who die in Sweden are in need of palliative care. The health services should offer qualified palliative care in line with the WHO's four basic elements, which include symptom relief, teamwork, communication and support for relatives. Based on these, the care must be designed individually adapted to the patient's wishes and needs. Nurses with their nursing responsibilies are those who for the most part meet patients in all wards of hospitals and have a leading role in palliative care.Aim: To describe nurses' experiences of palliative care of adult patients in hospitals.Method: The thesis was conducted as a literature review based on 18 scientific articles, with qualitative and quantitative approaches. The articles were obtained from the databases CINAHL and PubMed and were published between the years 2012 - 2021.Results: Lack of communication in the care team and with the patient and his or her next of kin was clearly prominent as an obstacle to adequate palliative care. Lack of knowledge evoked emotions such as insecurity and inadequacy in the encounter with the person at the end of life. The nurses' experiences of combining curative and palliative care constituted a lack of time and lack of space that had a negative impact on the quality of palliative care.Conclusion: Nurses testify to the lack of opportunities in performing palliative care with good quality in agreement with the basic palliative elements. Lack of time, knowledge and communication, are seen as obstacles in the performance of palliative care, which means that dying patients do not have the opportunity for a person-centered end of life.Overall, the results show that inpatient care in a hospital ward is not to prefer when it comes to palliative care.
