Bakgrund: När sjukdom inte längre är möjlig att bota övergår den kurativa vården till palliativ vård, som indelas i tidig och sen fas. Palliativ vård vid livets slut handlar om att få avsluta sitt liv med värdighet och utan smärta. Alla som är i behov av palliativ vård får inte det, trots att de behöver den. En brytpunkt krävs för att övergå till den sena palliativa fasen inom den palliativa vården. Syfte: Att beskriva sjuksköterskors uppfattningar om faktorer som påverkar palliativ vård. Metod: Detta examensarbete är en litteraturöversikt som bygger på 14 artiklar med kvalitativ ansats. Artiklarna är publicerade mellan 2012 och 2021. Resultat: Litteraturöversiktens resultatet visar ett tema och tre kategorier som påverkar palliativ vård. Temat är faktorer på främjar samt hindrar en god palliativ vård och kategorierna är; organisation, kommunikation, och beslutsfattande. Slutsats: Det finns både positiva och negativa faktorer som påverkar palliativ vård. Faktorerna finns på alla nivåer, från patient och närstående till sjuksköterskor och teamet samt på organisationsnivå. Kompetens, kommunikation och samarbete uppfattar sjuksköterskorna behöver utvecklas för att kunna ge en god palliativ vård. Det är många faktorer som ska stämma överrens för att det ska bli en palliativ vård som är förenlig med patienters och närståendes önskemål.

Background: When there is no possibility left to cure illness, curative care is transferred into palliative care, which is divided in an early and a late phase. Palliative care at the end of life is about to be able to die with dignity and without pain. Not everyone in need of palliative care receives it, although they need it. Information about transistion to end-of-life care are demanded to proceed to the late palliative phase. Aim: To describe nurses’ perception about factors that affect palliative care. Method: The thesis is designed as a literature review based on 14 articles with a qualitative approach. The articles are published between 2012 and 2021. Results: The literature review resulted in one theme and three categories affecting palliative care. The theme is factors that facilitate or complicate a good palliative care. The categories are; organization, communication and decision making. Conclusions: There are factors that are both positive and negative that affect palliative care. The factors are present at all levels; patients and relatives, nurses and the multidiciplinary teams and within an organizational level. The nurses perceive that competence, communication and cooperation need to be developed in order to provide good palliative care. There are many factors that have coincided in order to provide a palliative care that is compatible with the wishes of patients and relatives.
