Palliative Care from a nurse’s perspective in Nursing home : Literature Review (English)
Abstract [sv]

Bakgrund: Enligt Socialstyrelsen (2018) avlider cirka 90 000 personer årligen i Sverige. Cirka 80% av dem bedöms ha varit palliativt vårdade. Patienter inom palliativ vård finns inom både öppen och sluten vård samt inom kommunal hälso- och sjukvård. Palliativ vård är en komplex vårdform där flera olika vårdgivare kan behöva samarbeta för att patienten ska erhålla god personcentrerad vård.

Syfte: Syftet med denna studie är att beskriva sjuksköterskans upplevelser av palliativ vård på äldreboende.

Metod: Studien genomfördes i en litteraturöversikt. Litteraturstudiens resultat grundar sig på 12 kvalitativa artiklar och 1 mix-method. Sökningar gjordes i Cinahl och PubMed. Artiklarna granskades med hjälp av Örebro universitets granskningsmall och sedan analyserades med Fribergs fem-stegsmodell.

Resultat: Resultatet gav huvudkategorierna Sjuksköterskans utmaningar, Personcentrerad vård och Samverkan. Sjuksköterskorna saknade stöd från kollegor, tid för de äldre personerna och anhöriga samt brist på kunskap inom palliativ vård. Ekonomi och besparingar var ett annat hinder för att kunna ge en god palliativ vård och bevara de äldre personernas värdighet.

Slutsats: Avsaknad av samarbete i team, bristande kommunikation samt utbildning var hinder för en god vård. Sjuksköterskorna upplevde att de hade en hög arbetsbelastning och att de kände sig ensamma i den palliativa vården på äldreboenden.

Abstract [en]

Background: According to the National Board of Health and Welfare (2018), approximately 90 000 people die annually in Sweden. About 80% of them received palliative care and the sepatients are in both open and closed care as well as in municipal health care. Palliative care is a complex care generally with several caregivers who need to work together for the patient to receive good personcentred care.

Aim: The aim of this study is to describe the nurse experiences of palliative care in nursinghomes.

Method: The study was conducted in a literature review. The results are based on 12 qualitative articles and 1 mix method. Searches were performed in the databases Cinahl and PubMed. The articles were reviewed with help of Örebro University´s review template and then analyzed using Friberg´s five-step model.

Results: The results gave the main categories Nurse´s Challenges, Person centred Care and Collaboration. The nurses lacked support from colleagues, time for the elderly and relatives and a lack of knowledge in palliative care. Finances and savings were another obstacle to being able to provide good palliative care and preserve the dignity of the elderly.

Conclusions: Lack of cooperation in teams, lack of communication and training were barriers to good care. The nurses felt that they had a high workload and that they felt alone in the palliative care in nursing homes.
