Abstract [sv]

Bakgrund Palliativ vård innebär en helhetssyn av patienter som lever med en obotlig och livshotande sjukdom, där målet är att främja livskvalitet samt ge stöd till närstående. Palliativ hemsjukvård har blivit allt vanligare, vilket kräver att sjuksköterskor är självständiga och har kunskap inom arbetet för att utföra god och säker vård.

Syfte Syftet med denna litteraturöversikt var att belysa sjuksköterskors upplevelser av palliativvård i hemmet.

Metod Litteraturöversikten genomfördes som en strukturerad litteraturstudie med inslag av den metodologi som tillämpas vid systematiska översikter. Artiklarna söktes i databaserna CINAHL och PubMed och var publicerade mellan 2012–2022. Sökningen resulterade i 16 artiklar med kvalitativ ansats som analyserades med hjälp av Fribergs (2017a) femstegsmodell.

Resultat I resultatet framkom tre huvudkategorier med sex underkategorier byggt på sjuksköterskors upplevelser av palliativ vård i hemmet: sjuksköterskors roll, relationer och utmaningar. Resultatet belyser vikten av kompetens och teamsamverkan, goda relationer till patienter och närstående samt utmaningar med hemmiljön och tidsbrist.

Slutsats Hemmet upplevdes som en speciell plats att utföra vård på och det kunde vara svårt att anpassa vården utifrån patienternas hem. Det självständiga arbetet var utmanande och tidskrävande samtidigt som det upplevdes meningsfullt. För att känna trygghet i arbetet var fungerande teamsamverkan, stöd från kollegor samt kunskap inom området avgörande. En förtroendefull relation till patienter och närstående underlättade sjuksköterskornas dagliga arbete och var viktigt för att kunna utföra god palliativ vård.

Abstract [en]

Background Palliative care involves a holistic view of patients living with an incurable and life-threatening illness, where the goal is to promote quality of life and provide support to relatives. Palliative home care has become more common, which requires nurses to be independent and have knowledge in their work to provide good and safe care.

Aim The aim of this literature review was to illustrate nurses’ experiences of palliative care at home.

Method The literature review was conducted through a literature study that was structured with the element of methodology used in systematic reviews. The articles were searched in the databases CINAHL and Pubmed and were published between 2012–2022. The search resulted in 16 articles with a qualitative approach that were analyzed using Friberg’s (2017a) five-step model.

Results The result revealed three main categories with six subcategories based on nurses’ experience of palliative care at home: nurses’ role, relationships and challenges. The results illustrate the importance of competence and team collaboration, good relationships with patients and relatives, challenges with the home environment and lack of time.

Conclusions The home was perceived as a special place to provide care and it could be difficult to adapt care in patients’ homes. The independent work was challenging and time consuming, while at the same time it was meaningful. Effective teamwork, support from colleagues and knowledge were crucial to feel safe in the work. A trusting relationship with patients and relatives facilitated the nurses' daily work and was important to perform good palliative care.
