Abstract [sv]

Bakgrund: Tillförlitliga mått av fysisk aktivitet (energiförbrukning, hjärtfrekvens, stegfrekvens) är grunden för råd och rekommendationer för fysisk aktivitet som prevention och behandling. Dagens mätmetoder har dock kända mätfel. Objektiva mätmetoder, inklusive kommersiella produkter, har potential att bidra till ökad tillförlitlighet, men utvärdering av validitet bör föregå klinisk tillämpning.

Syfte: Att utvärdera Fitbit Versas kriterievaliditet gällande mätning av energiförbrukning (METS), hjärtfrekvens och stegfrekvens i laboratoriemiljö och i vardagsaktiviteter.

Metod: Kriterievalididet utvärderades genom beräkning av överensstämmelse och korrelation mellan Fitbit Versas och två guldstandardmätares mätningar av METS, hjärtfrekvens och stegfrekvens. Deltagare, vuxna (18–67 år) utan smärta rekryterades genom forskarens nätverk. METS, hjärtfrekvens och stegfrekvens samlades in i laboratoriemiljö (vila, gång på gångband) med Fitbit Versa, Jaeger Oxycon Pro och ActiGraph GT3X, samt med Fitbit Versa och ActiGraph GT3X under vardagsaktiviteter (METS, stegfrekvens). Förutom deskriptiv analys beräknades överensstämmelse med Intraclass correlation coefficent (ICC) och korrelation med Spearmans rank correlation coefficient.

Resultat: Studiens deltagare (n=38) var mellan 20 och 63 år (medel 42, SD 11.6) och 53%var kvinnor. Överensstämmelse mellan Fitbit Versas och Jaeger Oxycon Pros mätning avMETS i laboratoriemiljö var ICC 0.04 i vila, ICC 0.02 i 3 km/h, ICC 0.07 i 4.5 km/h, ICC0.07 i 6.0 km/h. Motsvarande korrelation mellan mätning av METS var som bäst 0.61 i 3.0km/h gång Överensstämmelse av mätning av HR var ICC 0.84 i vila, och under gångband ICC-0.52, ICC 0.29, ICC -0.07. Korrelationen av HR var som bäst 0.86 i vila. Överensstämmelse av stegfrekvens var i 3 km/h ICC 0.22, i 4.5 km/h ICC 0.24, i 6.0 km/h ICC 0.30,korrelationen som bäst 0.70 under gång i 3 km/h. Under vardagsaktiviteter var överensstämmelse för stegfrekvens som bäst 0.75, och för METS ICC 0.27.

Slutsats: Resultaten stödjer användandet av Fitbit Versa för mätning av stegfrekvens, men inte av METS och hjärtfrekvens.

Abstract [en]

Background: Reliable measures of physical activity (energy expenditure, heart rate, step frequency) are the basis for advice and recommendations for physical activity as prevention and treatment. However, today's measurement methods have known measurement errors. Objective measurement methods, including commercial products, have the potential to contribute to increased reliability, but evaluation of validity should precede clinical application.

Aim: To evaluate the Fitbit Versa's criterion validity regarding measurement of energy expenditure (METS), heart rate and step rate in a laboratory environment and in everyday activities.

Method: Criterion validity was assessed by calculating agreement and correlation between the Fitbit Versa's and two gold standard monitors' measurements of METS, heart rate, and step rate. Participants, adults (18–67 years) without pain were recruited through the researcher's network. METS, heart rate and step rate were collected in a laboratory environment (rest, walking on a treadmill) with Fitbit Versa, Jaeger Oxycon Pro and ActiGraph GT3X, as well as with Fitbit Versa and ActiGraph GT3X during everyday activities (METS, step rate). In addition to descriptive analysis, agreement was calculated with Intraclass correlation coefficient (ICC) and correlation with Spearman's rank correlation coefficient.

Results: The study's participants (n=38) were between 20 and 63 years old (mean 42, SD11.6) and 53% were women. Agreement between Fitbit Versa's and Jaeger Oxycon Pro's measurement of METS in a laboratory setting was ICC 0.04 at rest, ICC 0.02 at 3 km/h, ICC0.07 at 4.5 km/h, ICC 0.07 at 6.0 km/h. Corresponding correlation between measurement of METS was at best 0.61 in 3.0 km/h walking Concordance of measurement of HR was ICC0.84 at rest, and during walking ICC -0.52, ICC 0.29, ICC -0.07. The correlation of HR was at best 0.86 at rest. Concordance of stride rate was in 3 km/h ICC 0.22, in 4.5 km/h ICC 0.24,in 6.0 km/h ICC 0.30, the correlation being the best 0.70 during walking at 3 km/h. During activities of daily living, agreement for step frequency was at best 0.75, and for METS ICC0.27.

Conclusion: The results support the use of the Fitbit Versa for measuring step rate, but not METS and heart rate.
