Abstract [sv]

BakgrundBåde antalet personer som insjuknar i cancer och antalet personer som lever med cancer eller friskförklaras har ökat betydligt de senaste åren. Det betyder att många människor kan fortsätta leva sina liv efter att ha fått cancer och gått igenom cancerbehandling. Tidigare studier har visat att patienter med cancerdiagnos har upplevt att vårdövergångarna mellan olika vårdinstanser för cancerrehabilitering inte fungerat optimalt. Syftet med den här studien var att undersöka kontaktsjuksköterskors och fysioterapeuters erfarenheter av övergångarna inom specialistvård och till primärvård gällande fysioterapeutisk rehabilitering vid cancersjukdom.

MetodStudien är en kvalitativ deskriptiv intervjustudie. Kontaktsjuksköterskor (n=4) och fysioterapeuter (n=4) verksamma i slutenvården inom onkologi och cancerrehabilitering intervjuades angående deras erfarenheter av övergångarna mellan olika professioner och vårdinstanser gällande fysioterapeutisk rehabilitering. Intervjuerna transkriberades och analyserades enligt kvalitativ innehållsanalys.

ResultatAnalysen resulterade i två kategorier: Utmanande och tidspressade bedömningar och Samarbete och fragmentering i vården. Till dessa framkom sex subkategorier som var för sig beskrev olika utmaningar, rutiner, deltagarnas fokus samt vad som fungerar bra och vad som inte fungerar gällande övergångarna och samarbetet inom och mellan olika vårdinstanser.

SlutsatsResultatet visade på olika faktorer som påverkade rehabiliteringsvårdkedjan vid en cancerdiagnos med fokus på övergångarna mellan sjukvårdsinstanser. Bristande kommunikation och gemensamhetskänsla mellan instanserna samt tidsbrist var faktorer som påverkade processen kring vårdövergångarna. Sammantaget kan dessa faktorer påverka rehabiliteringskedjan negativt så att patienter med en cancerdiagnos inte får del av rehabiliterande insatser som skulle kunna bidra till bättre hälsa.

Abstract [en]

BackgroundNumber of people who get cancer, the number of people who live with cancer or are declared healthy from cancer have increased significantly in recent years. More cancer cases are detected at an early stage and treatments have become more effective. This means that many people can continue to live their lives after getting cancer and going through cancer treatment. Previous studies have shown that patients with cancer have experienced that the care transitions between different healthcare institutions for cancer rehabilitation did not function optimally. The aim of this study was to investigate the experiences of contact-nurses and physiotherapists in the transitions within specialist care and to primary care regarding physiotherapeutic rehabilitation in cancer.

MethodQualitative descriptive interview study. Contact-nurses (n=4) and physiotherapists (n=4) working in in patient care within oncology and cancer rehabilitation were interviewed regarding their experiences of the transitions between different professions and healthcare institutions regarding physiotherapeutic rehabilitation. The interviews were transcribed and analyzed according to qualitative content analysis.

ResultsThe analysis resulted in two categories: Challenging and time-pressed assessments and Collaboration and fragmentation in healthcare. In addition to these, six subcategories emerged that each described different challenges, routines, the focus of the participants and what works well and what does not work regarding transitions and collaboration within different healthcare institutions.

ConclusionThe results showed various factors that affected the rehabilitation care-trajectory in the event of a cancer diagnosis, with a focus on the transitions between healthcare institutions. Lack of communication and a sense of commonality between the institutions as well as lack of time were factors that affected the process of care transitions. Taken together, these factors can negatively affect the rehabilitation chain so that patients with a cancer diagnosis do not receive rehabilitative interventions that could contribute to better health.
