    Abstract [sv]

    Bakgrund: Vårdrelaterade infektioner och MRSA är ett stort problem i vården. Bristande följsamhet till vårdhygien är en orsak till smittspridning. Den legitimerade sjuksköterskan har ett personligt och arbetsledande ansvar för arbetet med evidensbaserad vård och för att gällande riktlinjer efterföljs. Syfte: Att belysa vilka kunskaper och förhållningssätt sjuksköterskor, sjuksköterskestudenter och vårdpersonal har till vårdhygien och MRSA samt vilka faktorer som påverkar följsamhet till vårdhygien. Metod: Litteraturöversikt. Resultat: Bristande kunskap och förhållningssätt är faktorer som påverkar följsamhet till vårdhygien och MRSA. Aktivt ledarskap och samlade strategier mot smittspridning påverkar följsamhet till vårdhygien positivt. Hög arbetsbelastning kan vara en negativ påverkansfaktor. Kunskapsbrist inom hela vårdorganisationen om vårdhygien och MRSA har identifierats. Bristande kunskap kan leda till låg följsamhet och felaktigt förhållningssätt. Bristfälligt förhållningssätt till evidensbaserade riktlinjer leder till att sjuksköterskor skapar egna hygienrutiner. Åtgärder som förbättrar följsamhet innefattar; goda hygienförebilder, förbättrad kommunikation, arbete för en engagerad personalgrupp samt att vårdpersonal får utbildning i vårdhygien och MRSA. Slutsats: En bristande kunskap om vårdhygien och MRSA har påvisats. Sjuksköterskan har som arbetsledare en viktig roll i att påverka annan vårdpersonal positivt. Ökad kunskap om vårdhygien och MRSA, ett positivt förhållningssätt, ett aktivt ledarskap och samlade strategier förbättrar följsamheten.

    Abstract [sv]

    Syfte: Att beskriva upplevelsen av att flytta från det egna hemmet och att bo på ett särskilt boende utifrån den äldres perspektiv.

    Metod: Studien genomfördes som en litteraturöversikt. De vetenskapliga artiklarna söktes via databaserna PubMed, Academic Search Elite, Cinahl och Vård i Norden, vilket resulterade i 14 artiklar. De vetenskapliga artiklarnas kvalitet granskades utifrån granskningsmallar och endast artiklar med medelgod eller hög kvalitet har använts i resultatet.

    Resultat: Förändrad boendesituation medförde en stor omställning för den äldre. Betydelsefullt för anpassningsförmågan var den äldres delaktighet i beslutet att flytta och förberedelse inför flytten. Många äldre var nöjda med omvårdnaden. De fysiska behoven som exempelvis förflyttning prioriterades av personalen, medan de sociala och psykiska behoven inte uppmärksammades i lika stor utsträckning. Personalbrist och inarbetade rutiner innebar begränsningar och liten möjlighet för den äldre att påverka det dagliga livet. Att känna trygghet, värdighet och bli bemött på ett respektfullt sätt av personal var viktiga faktorer för upplevelsen.

    Slutsats: Delaktighet i flyttningsbeslutet och förberedelse inför flytten var viktiga faktorer för den äldres anpassning till det nya boendet. Genom att personalen lyssnade på den äldre ökade förutsättningarna för att bemöta den äldres behov och önskningar. Individuellt anpassad omvårdnad där hänsyn tas till hela människan och dennes livsvärld, bidrar till upplevelsen av värdighet, livskvalitet och välbefinnande hos den äldre som bor på ett särskilt boende.

    Abstract [en]

    Background

    In Sweden, around 160,000 people live with a dementia diagnosis, which is a neurological condition where brain tissue has faded. This will lead to a decrease in physiological and cognitive abilities. Nursing interventions related to a person with dementia focus on relieving symptoms and providing assistance towards the functional decrease. These interventions can be applied to nutritional intake since meal situations can be a complex task for people with dementia. To accommodate the individual, these actions can be performed from a person-centered spectra.

    Aim

    The aim of the literature review was to explore person-centered nursing measures that a nurse can apply for various factors to optimize nutrition intake in people with dementia.

    Method

    An overview compiled of 11 scientific articles.

    Findings

    Five interventions that benefited the nutritional intake for people with dementia were listen to music during the meal, receive personal assistance to aid food intake, verbal prompting as support, how the food was presented, training programs at mealtime.

    Conclusion

    From this overview, interventions have been identified that are beneficial to the person with dementia in connection with nutritional intake. When no interventions are applied to meals, the condition of the person with dementia could be worsened dramatically through, for example, disease and immobility, resulting in deterioration in quality of life.

    The interactions presented could be provided by caregivers and relatives through simple approaches.

    Syfte: Att beskriva vilka faktorer som påverkar följsamheten till hygienrutiner hos vårdpersonal respektive patienter och dess påverkan på förekomst av vårdrelaterade infektioner. Metod: Examensarbetet utfördes som en systematisk litteraturöversikt med 24 vetenskapliga artiklar som grund till resultatet. Datainsamlingen gjordes i databaserna CINAHL with FULL TEXT och PubMed. Resultat: Huvudresultatet i litteraturöversikten har påvisat minskad frekvens av vårdrelaterade infektioner vid ökad följsamhet till hygienrutiner. Faktorer så som utbildning, påminnelser; visuella som hörbara, feedback; muntlig som skriftlig, motivation, olika typer av observationer och mätningar, ledarskap och ansvar, tillgänglighet av material samt implementering av förändringsstrategier, som implementerats enskilt eller i kombination, har sedan lett till ökad följsamhet till hygienrutiner hos vårdpersonal och patienter. Negativa faktorer för vårdpersonalens följsamhet rubriceras som hög arbetsbelastning, tidsbrist, personalbrist, okunskap, kontextuella normer samt glömska. Patientens empowerment, kunskap, samt följsamhet till handhygien visar att patienter kan stödja och stärka vårdpersonal i att utföra handhygien genom påminnelser. Slutsats: Faktorer som lett till ökad kunskap, ökad medvetenhet eller en kombination av dessa, har ökat följsamheten till hygienrutiner. Genom ökad följsamhet av hygienrutiner hos vårdpersonal och patienter har förekomsten av vårdrelaterade infektioner minskat.

    Abstract [en]

    Background: Trauma is the leading cause of death for people up to 40 years and

    causes great suffering for the affected person. When the person comes into the

    emergency department, it requires fast treatment and a structured approach from

    all of the trauma group. Every minute is important for the patient's prognosis. The

    encounter between the patient and the nurse is often short and intense.

    Aim: The purpose of this study is to describe patients' experience of care for

    physical trauma in an emergency department.

    Method: Articles have been applied for in the various relevant databases. Articles

    that answered the purpose was gathered and data were marked. This is to get an

    overview of the state of knowledge, which is compiled in the result.

    Results: The result shows that the communication and information were an

    important part of the care. Patients wanted the nurses to be honest and give clear

    information for patients, because it had impact on the experience of care. Feelings

    that arose in the connection with trauma was fear of the unknown, and not to hold

    control over their lives.

    Conclusion: Trauma care is a complex situation. The patient, purely somatic are

    in a life threatening situation but the mind is still present with many thoughts and

    questions. As the situation is life-threatening, the psychosocial needs will never be

    a priority of the traumacare. It will probably always remain, if no changes in the

    trauma team structure occurs.

    Postpartum depression in mothers of infants cared for in a Neonatal Intensive Care Unit: Incidence and associated factors2012In: Journal of Neonatal Nursing, ISSN 1355-1841, E-ISSN 1878-089X, Vol. 18, no 4, p. 143-51Article in journal (Refereed)
    Abstract [en]

    This longitudinal cohort study investigated the incidence of postpartum depression (PPD) among mothers of infants cared for in two Neonatal Intensive Care Units (NICU) and factors related to PPD onset. 123 mothers were posted the Edinburgh Post-Natal Depression Scale (EPDS) and a questionnaire to record infant and maternal data at 1 month, and a repeat EPDS scale at 4 months post-discharge. PPD incidence was 15% at 1 month, 14% at 4 months, and varied by NICU (23% vs. 8%). Pre-pregnancy and/or antenatal depression was significantly associated with the incidence of PPD. Mothers who experienced PPD at 1 month had an almost eight fold risk of experiencing PPD at 4 months. Women who were not offered counselling during their infant’s stay on the NICU had a 60% increased risk for PPD onset. The findings highlight the need for routine pre-natal screening and targeted support for mothers with infants admitted to NICU.

    Abstract [en]

    Background: Patients who survived critical illness or injury often experience physical and mental health problems during the recovery period. Weight/muscle loss and pain are common problems. The purpose of this study was to describe how weight loss affects health-related quality of life and to investigate the diagnostic groups that had the greatest weight loss and experienced reduced mental health and pain at the time of the follow-up visit to intensive care clinic.

    Method: A retrospective cross-sectional study in which 185 patients undergoing critical illness were followed up after hospitalization. Weight before ICU and at the time of follow-up was recorded. Health-related quality of life was assessed by health-survey SF-36. Data were presented by descriptive- and analytical statistics.

    Results: Men estimated significantly improved physical function compared to women while women estimated significantly better mental health than men. Patients with gastrointestinal disorders and trauma had a significantly greater weight loss compared to patients with lung-related conditions. Only patients aged 73 years or older had a significantly impaired physical function. Factors that affect physical function was pain and mental well-being. Patients with aortic aneurysms had significantly poorer mental health and more pain compared with patients suffering from gastrointestinal problems. Patients with aortic aneurysm and intracerebral- and lung-related disorders had significantly more pain than gastrointestinal- and trauma patients.

    Conclusion: By identifying the diagnostic- and age groups suffering most from weight loss, physical and psychological problems after intensive care measures can be initiated early and possibly physical and mental suffering reduced.

    Background

    : When it comes to heart disease, women are degraded. Women get worse treatment, more often wrong or no diagnosis and research is less in women. As the man is the norm and reference the women also get treated after the same criteria. Women’s symptoms are different compered to men.

    Aim

    : The aim of this study was to explore women’s experiences of suffering from a myocardial infarction.

    Methods

    : The design is a literature review. Data collection occurred in the databases Cinahl and PubMed which resulted in eleven qualitative articles.

    Result

    s: Women experienced symptoms from different parts of the body, which may be from the gastrointestinal area, extremities and breast region. They deny the symptoms and try to find reasons why they occur. Care provides give lack of information and does not take them seriously, which causes women to feel insecure. Women still feel bad several months after the infarction and have difficulty coping with everyday life. They need support from the family that can give them support and help, although they do not always tell family how they feel.

    Conclusion

    : Women experience a variety of symptoms when they suffer from myocardial infarction, and symptoms may start from different parts of the body, not just in the breast region. They also experience that the caregiver provides inadequate information and do not take them seriously when they seek medical care. Many women have remaining symptoms several months after the infarction.

    Abstract [en]

    The

    purpose of the study is to identify urinary incontinence work with quality registry Senior Alerts urinary incontinence module in two municipalities over a fixed period January to June 2014 and from January to June 2015. The method was to use the registry data that is later held by the Depositary of the Senior Alert. The result showed that more than half of the individuals in risk assessments were conducted on municipal G had any form of urinary incontinence in both periods. The municipality M proportion was even higher, about 80%. Underlying causes of urinary incontinence was found to be the aspects mentioned in the Senior Alerts module except constipation problems and loss of sensation in the genital area that is not revealed as clearly as the other aspects. Using Senior Alert, a pattern emerged, and it is possible to provide individually tailored measures for people with incontinence problems. With simple measures, can patients with feces regime, toilet assistance, testing of incontinence aids can the scale of Senior Alert be helpful. The scale demonstrates what assistance may be needed for patients, health professionals then perform the action.

    Syfte: Att belysa vilka faktorer som hindrade och möjliggjorde ett patientsäkert informationsutbyte mellan sjuksköterskor hos olika vårdgivare, vid in- eller utskrivning av äldre patienter.

    Metod: Uppsatsen utformades som en litteraturöversikt. Artiklar söktes i databaserna Cinahl, Pubmed och Web of science. Av de 14 artiklarna som användes var 8 kvalitativa, 3 kvantitativa och 3 artiklar hade en mixad kvalitativ/kvantitativ ansats.

    Resultat: Faktorer som hindrade ett patientsäkert informationsutbyte var: hög arbetsbelastning, bristfälliga rutiner, brist på kunskap, negativa attityder bland vårdpersonalen, brist på kontaktinformation samt information som var bristfälligt anpassad till mottagaren. Faktorer som möjliggjorde ett patientsäkert informationsutbyte identifierades som: Förbättrade rutiner, ökad samverkan mellan vårdgivare, att införa samordnare, bättre framförhållning, anpassad information till mottagaren samt att införa standardformulär.

    Slutsats: Ett patientsäkert informationsutbyte mellan hälso- och sjukvårdspersonal vid in- och utskrivning av äldre patienter, kräver ett omfattande samarbete mellan vårdgivare och ställer höga krav på sjuksköterskans kunskaper i kommunikation.

    Syfte: Att beskriva omvårdnadsåtgärder vid förstoppning hos vuxna.

    Design: En litteraturöversikt

    Metod: Artiklar till arbetet har sökts i databaserna PubMed och Cinahl. Resultatet

    har granskat, jämfört och sammanställt av författarna.

    Resultat: Författarna har funnit flera omvårdnadsåtgärder som visat signifikant

    förbättring av symtom vid förstoppning. De omvårdnadsåtgärder som undersökts i

    studierna är fibrer, rörelseträning vid toalettbesök, massage, probiotika, vätska och

    införande av protokoll där flera åtgärder visat signifikant förbättring vid

    förstoppning.

    Slutsats: Olika åtgärder har gjorts på flera olika patientkategorier där resultatet

    har varierat från objektiva fynd som exempelvis förbättrade tarmrörelser till

    subjektiva fynd så som ökad livskvalitet.

    Background:

    80% of the people that die every year in Sweden are believed to have benefited from palliative care. In order for patients to be involved in their care, they must be well informed about their condition. Reports show that not all palliative patients get a breakpoint conversation, which increases the risk that patients don’t feel involved in their care.

    Aim:

    To investigate patients’ experiences of participation in their end-of-life care

    Methods:

    The design is a literature review. Data collection has been done through searches in the databases PubMed and CINAHL.

    Results:

    The result was based on 15 scientific articles and were divided into four categories: patients’ need for information, shared decision-making, patients’ preferences, and experience of a good death. Most patients wanted information about their condition, but not everyone felt that they received it. It varied in how much patients wanted to be involved in the decision-making. Patients who wanted shared decision-making felt less that their wishes were met.

    Conclusions:

    To ensure that patients feel involved in their care, it is important to investigate the individual's needs and to adjust the care accordingly, as well as being aware that this may change over time.

    Abstract [en]

    Aim: The aim was to explore the experiences of recently graduated nurses during their first year of practice.

    Method: A literature review based on fifteen scientific articles that were sought out in the databases PubMed and CINAHL.

    Results: Six categories were identified: not feeling prepared for the profession, lack of support, how relationships with colleagues and physicians were experienced, bullying from peers, the feeling of not being able to live up to the ideals and transition. Most participants felt that the demands put on them when they entered the work force were high and they felt ill-prepared to meet them, and also that they received little support and wanted more support than they were getting. Slightly more than half of the studies described predominantly positive relationships with colleagues. One or more study participants was exposed to bullying in almost half of the articles. Almost all newly qualified nurses felt that it was difficult, stressful and frustrating to start working, but after about six months, they felt competent to handle routine tasks, more comfortable in the professional role, more independent and confident.

    Conclusion: The first few months were perceived as difficult and overwhelming, but after six months, most began to feel comfortable in the professional role.

    Abstract [en]

    Background:

    The elderly is a group that will be a major part of the population in the future. With increasing age, the risk of suffering from one or more diseases increases, which will place higher demands on healthcare. Swedish law states that healthcare has an obligation to promote patient integrity, participation and self-determination, but this is invalid in the care of the elderly. Many complex illnesses in an elderly person necessitate nursing based on the patient's individual needs. Person-centered nursing is about a partnership between the individual and healthcare staff, where the individual is at the center of care.

    Aim:

    To highlight the elderly patient's experience of care based on a person-centered perspective.

    Method:

    A literature review was conducted and the data was found in the databases CINAHL and PubMed, 13 articles with qualitative method were selected.

    Result:

    The results showed that there were similarities and differences in how the elderly patient experiences their care. These experiences can be divided into three main categories: self-determination, participation and integrity. The elderly patient had a longing for independence and self-determination, however, this was reversed due to a lack of time and competence shortage. The elderly had a desire to be involved in their care, where the relationship and communication were an important basis for creating participation. Many of the elderly did not feel confirmed as a unique individual of nursing staff.

    Conclusion:

    In order to promote the role of the elderly in their care, must a holistic perspective and a person-centered approach be applied, where the elderly is respected as a unique individual. With the older individual at the heart of his care, his right to self-determination, participation and integrity is strengthened, which in turn can lead to an increased quality of life.

Previous studies also illustrate various communication theories that are useful in healthcare relationships, but not so much regarding nurses’ experiences.

    Aim: To describe how nurses at outpatient psychiatric experience describe communication with adult patients with ADHD.

    Methods: The study had a qualitative design where data was collected through semi-structured interviews. Qualitative content analysis was used as analytical method.

    Results: The analyses resulted in one theme, four categories and eleven subcategories. Nurses at outpatient clinics experienced communication with patients with ADHD nourishing and debilitating at the same time. Various communication techniques were used in combination with the knowledge of disabilities to create the best conditions for communication with patients. Good attitude and a good approach were reported to be important.

    Conclusion: Nurses in outpatient psychiatry experienced communication with patients with ADHD as both nourishing and debilitating. The nurses adapt their communication to the patient’s needs and abilities, and they treat all needs of the patients' different with respect. Nurses felt that a good attitude and a good treatment was important for communication with patients with ADHD.

    Background: Hospitalization can bestow a feeling of autonomy deprivation and loss of abilities for the elderly persons. The elderly person expects to be seen by the nurse as an autonomous person and have their individual assets identified.

    Purpose: Purpose is to describe the nurses´ perceptions of the work with elderly persons´s assets during inpatient stay at a Geriatric ward.

    Method: Method is qualitative interview study with a phenomenographic methodology.

    Results: Nurses’ perceptions of the work with elderly person’s assets during inpatient stay at a Geriatric ward can be described in an overriding category and four categories. The overriding category was nurses have different understanding for the term assets. The four categories were; possibilities given to let de elderly person do what him/herself can do, the work with the elderly persons assets is sprung from a person-centered mindset, the elderly persons assets can be hidden and the elderly persons assets is not taken at use.

    Conclusion: Nurses’ percept an absence of work with the elderly persons assets despite theoretical awareness. Thru a person-centered way of working can the elderly persons assets be taken care of. As a nurse identify assets is a way of strengthen the elderly persons health and quality of life.

    Background:

    People with abuse and/or addiction are present in society and are an increasing group. Diseases due to abuse and/or addiction to alcohol or drugs leads to the need for care in different care units. Aim: The aim is to put together knowledge about the nurse experience of patients with abuse and/or addiction to alcohol or drugs in somatic care. Method: This literature review summarizes results based on 11 academic articles. Canada, USA, Taiwan, England, Australia, Chile and Israel are the countries represented in the results. The articles are published between 2008-2018. Results: Based on the reviewed articles, 4 categories emerged. The categories are nurses experience of care, attitude, lack of knowledge and support at the workplace. The result shows that education and workplace support is lacking. Education and workplace support is required to achieve a better attitude from the nurse. Conclusion: The conclusion of this literature review is that the nurse needs support and education in order to work with a good attitude towards patients with addiction and/or addiction to alcohol or drugs.

    Background: There is a great shortage of nurses. Newly graduated nurses are a group at high risk of being burned out and up to every fifth nurse is considering leaving the profession within its first five years. Many of the newly graduated nurses feel unprepared for the nursing profession and several studies show the gap between theory and practice.

    Aim: Describing challenges the newly graduated nurse experiences during the first year of working.

    Method: The study has been conducted as a literature review. With the help of the search databases CINAHL and PubMed, a total of 20 articles form the base for the result.

    Result: Several factors are described by newly graduated nurses as challenges. This includes a theory-practice gap, the lack of support and mentoring, a lack of knowledge, poor work environment, staff shortages and an increasingly complex care with patients have multiple ilnesses who require an expertise that the newly graduated nurse cannot possibly possess.

    Conclusion: The challenges presented in the literature review's results are well-known and global problems, but also influential. The focus should be on both appointing new nurses but more importantly valuing the ones already employed/working. Newly graduated nurses need better support when they enter the workplace, much also indicates that the education should be looked over in order to match the reality and prepare the students for work life as well as possible.

    Background: Yoga during pregnancy has become increasingly popular in Sweden and the western world in recent years. Earlier studies suggest that yoga for pregnant women may increase well-being during pregnancy, reduce fear for giving birth and increase prenatal attachment. Today, yoga for pregnant women is available at private yoga and fitness centres and at certain maternity clinics. However, there seems to be a gap of knowledge about midwifes experiences of working with yoga for pregnant women. Aim: Describe midwives experiences of working with prenatal yoga. Method: Eight semistructured interviews were conducted with midwives who have experience instructing yoga classes during pregnancy. Qualitative content analysis was used for analysis of the material. Result: Four main categories were identified. The categories were Birth Preparation, Well-being during Pregnancy, Prenatal attachment, and Contraindication or Adjustment. Furthermore, there were 11 subcategories and an overall theme: Sense of security and Trust. Conclusion: The midwives experience that yoga during pregnancy can contribute to increased well-being, both mentally and physically, establish prenatal attachment and prepare the pregnant women for childbirth. The midwives emphasize that their professional experiences in midwifery contributes to a deeper dimension in the yoga classes, enabling a safe practise during pregnancy. Clinical application: The study can inspire midwives to work with, or recommend prenatal yoga and inspire to further studies.

    Breast cancer is the most common cause of death in middle-aged women in Sweden after lung cancer. Treatment of breast cancer usually consists of surgery, where the affected breast is removed, mastectomy. Diagnosis with breast cancer and walking through mastectomy can lead to complex changes in the woman's identity, as well as suffering, anxiety and altered self-esteem. The purpose of the nursing care must be that the woman being cared for should be independent, and experience health. The nurse must have communicative skills to maintain person-centered care.

    Purpose:

    To compile knowledge of what experiences women with breast cancer have of nursing after completed mastectomy.

    Method:

    A literature review. The result is based on 14 scientific articles.

    Results:

    In summary, healthcare staff had difficulty meeting the needs of women after completion of mastectomy. The results show that the women had experience of inadequate information from healthcare professionals about rehabilitation and complications after mastectomy. Nursing staff lacked knowledge of complications that may arise after mastectomy to meet the needs of women. The results of the studies indicated that women lacked follow-up after mastectomy. The women were pleased with the emotional support of healthcare professionals and that the healthcare staff were supportive, provided information and that they were available for questions. The women wished the care staff to provide a person-centered approach.

    Conclusion:

    Both positive and negative experiences were found in the results of the literature review regarding women's experiences of nursing after mastectomy. Most women in the chosen studies felt that healthcare staff could not meet their needs.

    Aim: The aim was to illustrate relatives experiences and needs of support and professional approach from nurses associated with family members psychotic disorder.

    Method: This study was conducted as an empirical study with qualitative approach, based on semi-structured interviews. Relatives were asked to describe their previous experiences of support and professional approach from the nurse, when a family member suffers from a psychotic disorder. They also described which needs of support and professional approach they claimed to have.

    Results: The results showed that relatives of people with psychosis have no support from nurses and other health professionals. Experience of the nurse´s approach revealed deficiencies. The relatives felt extradited, rejected and without influence.

    . Relatives described their needs for support from the nurse to be relieved, valued and freed from guilt. They described a desire to be treated with care. It also emerged need to be more involved in the care of the family member, and to get advice and support. Mainly in the form of a contact person to turn to.

    Conclusion: Existing health care programs and guidelines need to be followed so relatives could receive the support they are entitled to. If the nurse involved relatives in health care, should relatives conditions for good health improve. It also contributes to relatives possibilities to support their sick family member to a better life.

    Background:

    Chronic leg ulcers are defined as wounds below the knee joint that has not healed within six weeks. The nurse has an important role in the care of patients with chronic leg ulcers, although the characteristics of the chronic leg ulcers can complicate the nurse’s work.

    Aim:

    The aim of this literature review was to describe which factors affect the quality of life in patients with chronic leg ulcers.

    Method:

    To fulfill the aim, a literature review was conducted based on 16 articles of both qualitative and quantitative approach, as well as the approach "mixed-method".

    Results:

    Chronic leg ulcers affected the patients’ quality of life from the physical, social and emotional perspective. Odour, pain, social and physical limitations and lack of coping ability have a negative impact on the quality of life. The nurse’s technical skills and ability to satisfy the patient’s need for trust and support influenced the quality of life of this patient group.

    Conclusion:

    Chronic leg ulcers often mean a deteriorated quality of life. The characteristics of the chronic leg ulcers often include odour and pain, which limits the patients’ daily life both socially and physically. These patients often experience feelings of anxiety, fear and impaired self-esteem related to how they and their leg ulcer are perceived by others, which also adversely affects their quality of life. The quality of life in patients with chronic leg ulcers is also affected by the nurse’s technical skills and ability to satisfy the patients’ need for support and a trustworthy relationship.

    ABSTRACT:

    Background: Work related stress among nurses contributes to a high percentage

    of sick leave. The work pace is often higher in the emergency care than in other

    work areas and the amount of people passing thru is high. Nurses working in

    emergency care are more exposed to stress. Coping strategies can be used to

    handle stress. Purpose: The purpose is to find out which stressors that has an

    impact on nurses health in the emergency care and which coping strategies that can

    be helpful to handle the stress. Method: Literature review of twelve qualitative

    and quantitative scientific articles regarding the nurse in emergency care, workrelated

    stressors and coping strategies. Results: Work-related stressors contributes

    to illness and suffering for the nurse in emergency care. Coping strategies like

    support from colleagues, debriefing, review of the workplace, and mindfulness

    contributes to that work-related stress is manageable and that health is achieved.

    Conclusion: Stressors such as lack of nurses, death or sexual abuse of children,

    violence and threats from patients and families, as well as deficiencies in the work

    environment contributes to work-related stress. Coping strategies may help to

    prevent work-related stress. But for coping strategies to work it is important to use

    the existing strategies and that time and opportunity are available at work to

    perform these.

    Background: Already in the 1970’s were significant others situation in the

    healthcare highlighted, and although there has been an increase on family-related

    research over the past 20 years, there are still gaps in family-related care.

    Aim: The aim was to describe significant others experience of needs from the

    nurse, when a patient suffer from a serious illness or injury.

    Method: The study was conducted as a literature review, which included 10

    articles of which 8 were qualitative and 2 were quantitative. Searches were made

    in Cinahl, PubMed and DiVA. The content was analyzed and themes were

    identified.

    Result: Two main themes and three subthemes were identified. The first main

    theme was the significant other’s needs, with the first subtheme being the meaning

    of communication in healthcare with the significant other and the second subtheme

    being recovery. The second main theme was available support, with the subtheme

    being significant others participation in care.

    Conclusion: The study implies that significant others felt a need to be involved in

    health care around the patient, especially by getting information. To involve the

    significant other can reduce the stress experience, which in turn can raise the trust

    in health care.

    Background:

    Continuous midwifery-led care is a concept based on the assumption that

    women who receive care the same midwife during pregnancy, childbirth and postpartum care

    will take an active part in their own care and feel a strong sense of satisfaction and personal

    control.

    Aim:

    To describe the knowledge of women's experiences of continuous midwifery-led care,

    from early pregnancy to being a new parent.

    Method:

    A literature review was conducted with 12 articles searched through Cinahl och

    Pubmed.

    Results:

    Continuous midwife-led care contributed to women taking active decisions

    regarding their care, feeling a greater sense of self-esteem and experiences an increased

    confidence before childbirth. The study highlighted the importance of a good relationship,

    clear information and that they had time to describe their wishes before delivery. The women

    felt that the continuous midwifery-led care was respectful and accessible and gave them

    emotional support and where conducted in a safe and competent way.

    Conclusion:

    Continuous midwifery-led care during maternity care, childbirth and postpartum

    periods can strengthen women's sense of participation in care, positively affect the

    relationship between the woman and midwife, and strengthen the woman's self-esteem before

    birth.

    Background: Globally, dementia is considered one of the major diseases of the population. Symptoms of dementia are divided into cognitive, behavioral and physical. These cause severe suffering to the person with dementia and is a challenge for the relatives and healthcare professionals. Care and nursing for persons with dementia should, according to the National Board of Health and Welfare, be given from a person-centered approach. Previous research shows that person-centered care is recommended for providing high-quality care. The nurse's responsibility is nursing. The consensus concept of nursing is a human being, health, nursing and the environment. Previous research shows that music can be a useful in the care of people with dementia. Purpose: To present research describing how music is used in dementia care and demonstrate the possible effects on people with dementia.  Method: Literature review to compile the state of knowledge. Articles were searched in the databases PubMed, CINAHL and PsycINFO with the search words "dementia", "music", "care" and "nurs *". After the quality review, 16 articles were included. The articles were analyzed, interpreted and compiled in a result matrix. Result: The result of the literature review showed that music as part of the nursing and care had a positive effect on the symptoms associated with dementia. The most significant positive effects were found in the four areas of behavioral, psychological, cognitive symptoms and quality of life. Conclusion: Music is a good alternative and/or supplement in nursing and treatment of people with dementia.

    Aim: The aim of this study was to describe nurses' experiences and strategies in situations of difficult conversations with patients at the end stage of life.

    Method: The study was conducted as a literature survey. The result is based on 14 articles of quantitative and qualitative approach, which were collected from CINAHL and PubMed.

    Results: Nurses experienced conversations with patients at the end stage of life as a difficult task to deal with. The lack of knowledge were the main factors that affected the implementation of difficult conversations. The nurses used different strategies in order to help the patient in the best way possible. The nurses had strategies that were about to try to build a relationship with the patient. They prepared themselves before they would meet with the patient to have such a great picture of the patient's situation as possible. The attitude towards the patient could adapt depending on the patient they met. Some nurses had as a strategy to avoid the difficult conversations with patients at end of life. It is because of a lack of knowledge about the management of difficult conversations.

    Conclusion: The lack of knowledge influenced the nurses handling of difficult conversations, thus it is highly relevant that the opportunity of additional theory is available. Practical knowledge can be obtained by colleagues and other workers within the profession, which can give nurses the opportunity to further develop strategies to improve the handling of difficult conversations.

    Background: Attention Deficit/Hyperactivity Disorder (ADHD) is an increasingly common diagnosis among adults. Nurses need to have the ability to be able to meet this patient groups needs.

    Aim: The purpose is to describe nurses’ experience in care of patients with cognitive difficulties with focus on ADHD.

    Method: The design is a literature review of 9 qualitative and quantitative articles. The collection of data has been done via the databases Cinahl, Psycinfo and PubMed.

    Result: The review of the articles resulted in two main themes: Conditions and limitations in nursing care in patients with cognitive difficulties as ADHD with three subtopics Time, Treatment and Attitudes and Communication between nurse and patient and care units with two subtopics; Communication between nurse and patient and Communication between care units. The results show that conditions and limitations for example time, attitudes and relationships affected nursing in care of patients with cognitive difficulties as ADHD. There could be negative attitudes among healthcare professionals. However, a therapeutic relationship and communication between a nurse and a patient was found to benefit nursing patients with cognitive difficulties such as ADHD.

    Conclusion: Communication was an important factor in the care of patient with ADHD and the nurse had an important role in this. Time and the patient’s conditions could affect the nurses’ potential of communication.

    Background: How the midwife treats the women impacts her experience of childbirth. Women with GDM experience the diagnosis as stressful and worrisome, time-consuming and they wish for a pregnancy without GDM. The number of studies about their childbirth experience is low. Aim: A comparison of the self-assessed childbirth experience between women with a normal pregnancy and women with a GDM-diagnosis. To identify factors that can impact on the childbirth experience. Methods: A retrospective cross-sectional study. Data was collected thru a survey, using the VAS-scale, and registerdata from MFR. A total of 391 participated, 326 with a normal pregnancy and 65 with GDM. The material was analyzed using parametric and non-parametric statistics. Result: A total of 36,1% had a negative childbirth experience. It was more common within the GDM-group (55,4%) to have a negative experience than for women with a normal pregnancy (32,2%). Factors correlating with a negative experience was, a high BMI, born outside of Sweden, primiparous and if the women were unsatisfied with the provided pain-relief. Conclusion: Women with a GDM-diagnosis has a significantly more negative childbirth experience, factors related to pregnancy and childbirth showed a correlation with a negative childbirth experience. Clinical application: The results can be used to improve healthcare for women with GDM to contribute to a more positive childbirth experience.

    Background In person-centered care the patient and his/her experience of illness, disease, social content and close relationships is central. Person-centered care emanates from three keywords developed at University of Gothenburg Centre for Person-Centered Care (GPCC): Partnership, the patient narrative and documentation. Purpose To describe the experiences nurses have from person-centered care and how they work with partnership, the patient narrative and documentation at general units in nursing homes. Method Descriptive study conducted with a qualitative deductive approach. The study was based upon eight semi-structured interviews and data was analyzed using qualitative content analysis. Results One theme was identified: The communicative conversation used as a tool in order to perform person-centered care. Three categories were predetermined and the analysis resulted in seven subcategories: Partnership; to utilize the patient´s knowledge, to utilize the knowledge held by relatives, caregivers and the nurse, The patient narrative and Documentation; documentation of the story of the patient´s life, documentation in individualized care-plans and documentation in the system of documentation from a personal- centered perspective. Conclusion The communicative conversation was essential in order to perform person-centered care. Within this conversation, the nurse will be able to develop a relationship with the patient and from that relationship a partnership will evolve. In addition, a communicative conversation is of crucial importance in order to evolve relationships with relatives and to maintain functional cooperation. However, it turned out that the nurses experienced that they worked too far from the older person and that they did not always have time for in-depth conversations.

    Abstract [en]

    Background:

    Research suggests that care based on a person-centered basis

    contributes to a higher quality of care, and has a positive impact on the patients and

    the healthcare profession. At the same time, studies point out that health care is facing

    a variety of challenges in order to introduce a person-centered approach into daily

    care.

    Aim:

    To describe which barriers prevent the nurse to work person-centered in the

    healthcare meeting with the patient.

    Methods:

    The study was conducted as a literature review, the result is based on 16

    scientific articles.

    Results:

    Several factors that may constitute difficulties for the nurse to work personcentered

    in the healthcare meeting with the patient were identified and generated four

    categories:

    Collaboration and Teamwork, Attitude and Culture, Time and Resources,

    and

    Communication and The patient Narrative. The result indicates cooperation

    difficulties between different health care professions, contradictory attitudes and

    routines that not lie on a scientific basis. Lack of educational opportunities and

    knowledge about what person centered care means and how a person-centered

    approach should be applied in clinical practice. High workload and lack of time

    affected the nurse's ability to establish a relationship with the patient, and contributed

    to stress and feelings of inadequacy.

    Conclusion:

    Summary the literature study indicates that there are several barriers that

    make it difficult for the nurse to work person-centered in the meeting with the patient.

    In order for a person-centered approach to be practiced, active work is required

    individually and collectively at different levels within an organization to become part

    of the daily practice.

    Syftet: Syftet med denna litteraturöversikt var att beskriva föräldrars upplevelse av stöd från hälso- och sjukvårdspersonalen då deras barn diagnosticerats med cancer. Metod: Examensarbetet genomfördes som en litteraturöversikt. Vetenskapliga artiklar söktes i Högskolan Dalarnas biblioteks prenumererade databaser CINAHL och SAGE Journal Online. Av de 12 vetenskapliga artiklarna som användes var fem av kvalitativ ansats, tre var av kvantitativ ansats och fyra var av mixad metod. Resultat: Resultatet visade att föräldrar till cancerdiagnosticerade barn behövde olika typer av stöd, så som känslomässigt, psykologiskt, socialt samt avlastning. Samtalsgrupper behövdes för att föräldrarna skulle få träffa andra i liknande situationer och utbyta erfarenheter. Slutsats: Hälso- och sjukvårdspersonalen bör tänka på hur kommunikation används och information ges. Varje förälder ses som unik och bör därför bemötas individuellt. Föräldrarna behövde stöd i olika former för att kunna bearbeta sitt barns cancerdiagnos. Hälso- och sjukvårdspersonalen bör sträva efter en fungerande relation till barnet och föräldrarna för att skapa förtroende och en förutsättning för bästa möjliga vård.

    Background: The child health nurse has an important role to act early when suspecting child maltreatment. They also have a legal obligation to report such case to the social services. This is similar to other professionals who meet children in their duties. Nevertheless, previous research shows a varying compliance to this obligation.Aim: The aim of this study was to describe child health nurses’ experiences to identify signs and report to child health services when suspecting child maltreatment.Method: Qualitative content analysis based on five semi structured interviews with nurses working with child health.Results: To identify and report when suspecting child maltreatment where found to involve worrying about a child, working together whit colleagues and other professions and to have a good relationship with the family. It also involved focusing on the children’s best interests, to be affected on a personal level and having to alert the social services and cooperating with them. Cooperating with colleagues within the child health care was highlighted as very important. The most prominent in the contact with social services was difficulties and frustration over the lack of feedback and the desideratum of an improved cooperation.Conclusion: Previous research shows a varying compliance to the legal obligation to report when child maltreatment is suspected. It has been found that there are barriers that complicates the child health nurses’ work. This study found that a deficient cooperation between the social services and the child health nurses could form such barrier. It was also found that support from colleagues and a close collaboration with other professions within child health services was of great importance in efforts to identify and report when suspecting child maltreatment.

    Background:

    Healthcare-related infections are the most common healthcare injuries and lead to increased patient suffering, prolonged hospital stays and higher costs. Healthcare-associated infections can be prevented by healthcare personnel through compliance with basic hygiene routines. Although there are current regulations regarding this, the compliance of healthcare personnel is low. It is therefore important to find out what factors can give the staff an increased compliance to basic hygiene routines.

    Aim:

    The purpose of the literature review was to describe possible factors that may affect the compliance of healthcare personnel with basic hygiene routines focusing on hand hygiene. Method: The method of this study has been formed as a literature review and it is based on 13 scientific articles of both qualitative and quantitative methods, which were aquiered by search from the databases Cinahl, PubMed and PsycInfo.

    Findings:

    The results showed that there are several factors that affect the healthcare personnel’s compliance with basic hygiene routines. Particularly prominent factors that provide increased compliance were knowledge, training and placement of hygiene materials and sinks. Time, stressful and acute situations also affected compliance. Conclusion: Compliance to hand hygiene is low, despite current regulations and national efforts. There are several factors to consider in order to increase the compliance of healthcare personnel with basic hygiene routines, and this literature review can contribute to increased knowledge of these factors.

    Background: Each year around 64 000 people are diagnosed with cancer and tumor

    diseases are the second common cause of death in Sweden. Getting the diagnosis of cancer

    means a stressful time both physically and mentally. Nurse’s role is to alleviate suffering and

    promote god health. Touch is a universal human need and becomes particularly clear when a

    deadly disease affects a person.

    Aim: The aim is to illustrate the experiences of touch treatment of nurses and patients with

    cancer.

    Method: A literature review, fourteen research papers. Five qualitative and nine quantitative.

    Results: The result was divided in two areas: patient’s experiences of touch treatment and nurse’s experiences of touch treatment. In the area of patients experiences two categories

    showed: Soothing effect and supportive effect as well as subcategories: physical discomfort,

    physiological discomfort, well-being and closeness. In the area of nurse’s experiences

    emerged one category: Education in touch therapy as well as the subcategory experiences of

    touch therapy.

    Conclusion: Touch has a short-term symptom relief effect of pain, anxiety, nausea and

    provides increased well-being. Therewith the physical contact between nurse and patient

    improved and led to that the existential loneliness was reduced. The nurses saw the patient as a person instead for a patient with a disease.

    Abstract [en]

    Background: Colic affects at least 10% of newborns and parents seek help. The aim was to present why infants whose parents sought help for colic, where not included in two trials studying the effect of acupuncture in infantile colic and to describe the use of simethicone and probiotics amongst those infants that according to a diary still fulfilled the criteria for infantile colic after a baseline week with cows milk free diet.

    Method: A quantitative, descriptive sub-study based on unpublished data, collected in two RCTs, examining the effect of acupuncture in 228 infants with colic. Screening lists and background data from a questionnaire answered by the infant’s parents was analyzed.

    Results: 636 infants were screened for participation in the two trials. Of these, 389 were not included as they according to the diary cried < 3 hours per day and thereby did not fulfill the criteria for colic. Either parents had overestimated the crying or infants reduced symptoms of colic when introducing a cow’s milk free diet. Almost every included infant in the first trial (2010) and 76% in the second (2015) had been treated with simethicone but only 2,7 % and 5,4% of the parents reported effect. The use of probiotics increased from 14,8% to 86,3% in the second trial. Zero and 3,2% respectively reported effect. There was a significant association between treatment with simethicone and having more than one child, having earlier experience from colic in the family and having a normal delivery. Treatment with probiotics was associated with higher frequency of complications during pregnancy, exclusive breastfeeding, food allergies and/or intolerances in the infant’s family and having less problems with eczema and skin rashes.

    Conclusion: The use of a diary is valuable to assess the infants crying and can help nurses to adapt their advice and support to these families. Excluding cows milk protein helped many infants with excessive crying. Most parents had tried simethicone and probiotics without experiencing effect.

    Background: Type 2 diabetes increases globally, mainly due to an unhealthy lifestyle. Through health promotion interventions type 2 diabetes can be prevented and avoided.Aim: The aim of this study was to describe the experiences of persons with type 2 diabetes with respect to health promotion.Method: This study was a literature review based on 15 scientific articles within current subjects. Selected articles were reviewed and summarized in the categories Experiences about health promotion and Experiences regarding advice about health promotion.Result: Persons with type 2 diabetes experienced difficulties in changing their dietary habits and phyiscal activity habits. Knowledge of health promotion measures existed, however, they lacked the motivation and knowledge about how to achieve them. Advice and support from healthcare professionals, related parties and group education was an important factor in the implementation of health promotion measures.Conclusion: Persons with type 2 diabetes need support from different parties in order to implement health promotion measures. Changing their dietary and physical activity habits has sometimes been perceived as difficult. With support from health care, close relatives and group training, strategies for health-enhancing changes can be created. By meeting people in the same situation, people with type 2 diabetes can get a positive exchange of knowledge.

    Background. Evidence-based practice (EBP) is a worldwide approach to improving health care. There is, however, a shortage of studies examining whether or not newly graduated health care professionals are actually applying EBP in their daily work.

    Objectives. To examine the application of EBP in clinical practice by registered nurses (RNs) 2 years post graduation and to explore whether the application of EBP differed with regard to the clinical settings where RNs were working.

    Method. A cross-sectional design using a national sample. Data were collected in 2007 from 987 RNs (response rate 76%). Six items measuring respondents' self-reported extent of applying EBP were used.

    Results. Of the 987 RNs, 19% formulated questions and performed searches in data bases, 56% used other information sources, 31% appraised the literature, 30% participated in practice development and 34% participated in evaluating clinical practice. A greater proportion of the RNs working in elder care applied EBP compared with the RNs working in hospitals, psychiatric care and primary care.

    Conclusions. The RNs applied the components of EBP to a rather low extent 2 years post graduation despite EBP being an important objective in Swedish health care and educational programmes since the 1990s. These findings support other studies reporting the implementation of EBP in organizations as a complex and often slow process. The differences in the RNs extent of applying EBP in relation to their workplace indicate that contextual factors and the role of the RN in the organization are of importance for getting EBP into practice.

    Barriers to research utilization and research use among registered nurses working in the care of older people: does the BARRIERS scale discriminate between research users and non-research users on perceptions of barriers?2008In: Implementation science : IS, ISSN 1748-5908, Vol. 3, p. 24-Article in journal (Refereed)

    BACKGROUND: One strategy to enhance research use and change current practice is to identify barriers and then implement tailored interventions to reduce these barriers. In nursing, the BARRIERS scale has been frequently used to identify nurses' perceptions of barriers to research utilization. However, this scale has not been applied to care of older people, and only one study has investigated how identified barriers link to research utilization. Therefore, the purpose of this study was twofold: to describe RNs' perceptions of barriers to and facilitators of research utilization and to examine the validity of the BARRIERS scale in relation to research use.

    METHODS: A cross-sectional survey design was used and registered nurses (RNs) working in the care of older people participated (response rate 67%, n = 140/210). Two questionnaires, the BARRIERS scale and the Research Utilization Questionnaire (RUQ), were used. Data were analyzed using descriptive and bivariate inferential statistics.

    RESULTS: Characteristics of the organization and the presentation of research findings were rated as the most prominent barriers. The three items most frequently reported as barriers were: the nurse is isolated from knowledgeable colleagues with whom to discuss the research (89%); the facilities are inadequate for implementation (88%); and, the relevant literature is not compiled in one place (81%). Surveyed RNs suggested more support from unit managers and better availability of user-friendly reports in Swedish to enhance research use.The RNs reported a modest use of research. A weak but significant correlation was found between the Research Use index in RUQ and the Presentation subscale in the BARRIERS scale (r = -0.289, p < 0.01), suggesting that the RNs reporting more research use were less likely to perceive presentation of research as a barrier. Dividing the sample into research users (n = 29) and non-research users (n = 105), the research users rated significantly lower on the subscales Presentation, Nurse and Research in the BARRIERS scale.

    CONCLUSION: The BARRIERS scale revealed differences in the perception of barriers between research users and non-research users. Thus, methodologically the scale appears useful in identifying some types of barriers to research utilization but not organizational barriers. The identified barriers, however, are general and wide-ranging, making it difficult to design useful specific interventions.

    AIMS: To describe registered nurses' reported use of research in the care of older people and to examine associations between research use and factors related to the elements: the communication channels, the adopter and the social system.

    BACKGROUND: Research use among registered nurses working in hospital settings has been reported in many studies. Few studies, however, have explored the use of research among registered nurses working in the care of older people.

    DESIGN: A cross-sectional survey.

    METHODS: In eight municipalities, all registered nurses (n = 210) working in older people care were invited to participate (response rate 67%). The Research Utilisation Questionnaire was adopted. Questions concerning the work organisation and research-related resources were sent to the Community Chief Nurse at each municipality. Descriptive statistics and logistic regression were applied.

    RESULTS: The registered nurses reported a relatively low use of research findings in daily practice, despite reporting a positive attitude to research. The registered nurses reported lack of access to research reports at the work place and that they had little support from unit managers and colleagues. Registered nurses working in municipalities with access to research-related resources reported more use of research than registered nurses without resources. The factors 'Access to research findings at work place', 'Positive attitudes to research' and 'Nursing programme at university level' were significantly associated with research use.

    CONCLUSIONS: There is a great potential to increase registered nurses' use of research findings in the care of older people. Factors which were linked to the communication channels and the adopter were associated with research use.

    RELEVANCE TO CLINICAL PRACTICE: Strategies to enhance research use should focus on access to and adequate training in using information sources, increased knowledge on research methodology and nursing science and a supportive organisation.

    RATIONALE AND OBJECTIVE: Evidence-based practice is a strategic ingredient in today's health care. Despite extensive efforts to produce and disseminate clinical guidelines, research uptake is still a difficult task. In Sweden, elderly care (EC) has shifted from hospital care to community-based care, and the major nursing-staff group in EC has no university education. These and other factors make implementation of evidence-based care particularly challenging in EC settings. The purpose of this study was to identify determinants of research utilization in EC.

    METHOD: Two questionnaires that cover research utilization and organizational climate were mailed to all staff (n = 132) working in seven EC units. The response rate was 67%.

    RESULTS: Of all respondents, 28% reported that they used research findings in daily practice (the RU group). Remaining respondents constituted the non-RU group. Significant differences existed between the RU group and the non-RU group as per six individual and six organizational factors. Using logistic regression models, four factors were significantly related to research utilization, namely: attitudes toward research (OR = 5.52, P = 0.004); seeking research that is related to clinical practice (OR = 5.56, P = 0.019); support from unit manager (OR = 4.03, P = 0.044) and access to research findings at work place (OR = 6.65, P = 0.005).

    CONCLUSIONS: Individual and organizational factors were associated with the use of research in EC. Despite distinguishing conditions in EC settings, identified factors reflect well-known determinants of research use that, as in many other health care contexts, should be considered in the endeavours of evidence-based practice.

    Background.  Sweden has one of the largest proportions of older people in the world. To manage the healthcare needs of an aging population, there has been an alteration from hospital care to community-based care. In these settings, the majority of staff is enrolled nurses (EN) and nurse aides (NA) without university education. Aim and design.  The overall aim of this cross-sectional survey was to explore staff perceptions of factors related to research utilization in the care of older people. Method.  Questionnaires covering research utilization and demographics were sent to all staff (n = 132) working in seven units in older people care. The response rate was 67% (n = 89). The respondents consisted of ENs/NAs (n = 63), Registered Nurses (RN) and rehabilitation professionals (RP) as physiotherapists and occupational therapists (RN/RP n = 26). Results.  Most of staff reported positive attitudes towards research. The RNs/RPs stated more often than the ENs/NAs that they wanted to base their practice on research (81% vs. 25%; P = 0.001). The RNs/RPs also reported a greater extent of research use in daily practice (54% vs. 17%; P = 0.001). Support from colleagues (77% vs. 22%; P < 0.001) and unit managers (73% vs. 10%; P < 0.001) for implementing research findings was also more frequently reported by the RNs/RPs compared with the ENs/NAs. The majority of the ENs/NAs stated Do not know on many items concerning attitudes towards research, support for research utilization and actual use of research. Conclusions.  Despite overall positive attitudes towards research, the majority of staff did not use research findings in daily practice. This was particularly valid for the EN/NA group. Relevance to clinical practice.  There is an urgent need for managers and others in the care of older people to develop strategies for implementing evidence-based practice that involves the EN/NA group.

    Abstract [en]

    Objective

    The aim of this study was to examine the impact on asthma control and lung function of a web-based support program for asthma patients in primary care.

    Method

    The study was a quantitative, parallel, 2-arm, randomized controlled trial over a 6-month period.

    Both groups received asthma care based on how it was conducted at the health centers and the intervention group also gained access to a newly created web-based support program (Astmakollen) for asthma patients. In total, 83 participants completed the study, of whom 39 were in the intervention group and 44 in the control group.

    Results

    This study has showed significant improvement in asthma management in the sample group who had access to the web-based support program for 6 months. The mean score for asthma control test in the intervention group increased by 2,23 points while the score in the control group remained almost the same. The effect size for this change measured in Cohens

    d was 0,57. The mean score of lungfunction in the intervention group increased by 0,3% during the study period, while it deteriorated in the control group and the effect size amounted to 0,45. The difference between the groups were significant.

    Conclusions

    In this study, it appears that eHealth can provide improved asthma control and maintained lung function and be a complement in today's asthma care by increasing accessibility of personalized and evidence-based care.

    Background:

    Namibia has challenges in the area of sexual and reproductive health and rights. Working with sexual and reproductive health care, midwives are an important facilitating factor for a better sexual and reproductive health in Namibia.

    Aim:

    To explore midwives’ perceptions of HIV-positive women's sexual and reproductive health and rights in urban Namibia.

    Method:

    A qualitative study with ten midwives at a hospital in Windhoek. Individual interviews were done using an interview guide. The method used for analysis was phenomenography. The phenomenon being studied was the sexual and reproductive health and rights of HIV-positive women and the phenomenon was studied through the perceptions of midwives.

    Findings:

    The midwives perceived that improvements of HIV-positive women’s reproductive health were obstructed. The midwives perceived that the HIV-positive women’s reproductive possibilities and decisions were related to HIV. According to the midwives’ perceptions, sexual relations could be complicated by HIV and it was difficult for HIV-positive women to use condoms.

    Conclusion:

    The findings of this study can be used to reflect on the current situation for HIV-positive women in Namibia. The midwives' perceptions highlight the need to reduce the remaining stigma related to HIV and to work towards the empowerment of HIV-positive women.

    Background: The demands and expectations on a nurse in the emergency room

    are high. They are responsible for many patients and have to make many decisions

    on their own, sometimes with very short reflection time. They face a wide variety

    of patients with different problems and personalities, each requiring their own

    method of approach and interaction in order for him or her to feel acknowledged.

    Aim: The aim of the literature review is to examine patient's experiences of being

    cared for in an emergency room.

    Method: Articles for the work has been searched in different databases. Articles

    in-question has been read and the relevant finding has been highlighted, to analyze

    and compile the results.

    Results: The analysis resulted in three subheadings: Information, Treatment and

    Care environment, with sectioning of positive and negative experiences. Many

    patients were satisfied with the care, but had low expectations from the start.

    Conclusion: There is much that needs to be changed to allow patients to have a

    more positive experience of the emergency room. Care units must work harder

    with to ensure that patients will feel secure and safe, for example, better

    information on waiting times and a treatment where the nurse sees the whole

    patient.

    Doulor/kulturtolkar är kvinnor som ska ge sina kvinnliga landsmän stöd och hjälp i

    kontakten med den svenska hälso- och sjukvården. I Örebro läns landsting

    startades ett projekt med doulor/kulturtolkar under 2013. Syftet med denna studie

    är att utforska dessa doulors erfarenheter av sitt arbete och vilken information de

    anser är relevant för invandrarkvinnor som ska besöka en barnmorska i

    primärvården. Metoden har varit kvalitativ med individuella intervjuer med fem

    doulor i Örebro. Resultatet visade att invandrarkvinnor önskar stöd och

    information dels i form av hjälp att förstå språket och information i samband med

    besök i vården t.ex. på vårdcentral, dels i form av en djupare förklaring av

    innebörden i informationen. Doulornas uppgift har varit att förklara på ett sätt så

    att invandrakvinnorna förstår. Invandrarkvinnorna har också sökt stöd hos

    doulorna när de har haft oro angående kvinnlig könsstympning, och för att få veta

    vad cellprovtagning är. Slutsatsen är att denna studie visar överensstämmelse i

    hög grad med tidigare forskning inom området.

    Abstract [en]

    Purpose: To compile the current studies on patients' experience and perception of bedside report in hospital care.

    Methods: The literature review was based on 16 scientific articles, eight qualitative, seven quantitative and one mixed methods. The articles were searched through databases Cinahl and PubMed and analyzed using content analysis, five themes were created based on the analysis.

    Results: Five themes were created based on the result: treatment and communication, expenditure of time, participation, safe and secure care and confidential information.

    Patients were generally positive to the method and felt that they were involved in their care. They stated that the transfer of information between nurse and patient was an important aspect that contributed to a sense of participation and safe care. Lack of time during the bedside reporting and that the nurses used an incomprehensible language were deficiencies raised by the patients related to the method.

    Conclusion: It is important that the nurses strive to create a balance of power in the relationship with the patient. This can be achieved by giving time and to adapt the language to the unique patient. This literature review can provide an understanding of how patients can experience bedside report and the factors that promote and inhibit participation. The result can be used as a guide in the introduction of the method in clinical practice.

    Background: Postoperative pain is often undertreated. There are several

    reasons why it is important with good pain relief after surgery. Ease of the

    acute pain but also to prevent complications and long lasting pain.

    Aim: To describe the nurse´s assessment and nursing care of postoperative

    pain in adults.

    Methods: Literature review based on 16 articles, published between 2010-

    2016. The articles were found in the databases PubMed, CINAHL and Web

    of Science.

    Results: The nurse´s attitude and time for creating a relationship with the

    patient, is a prerequisite for correct pain assessment. Apart from

    administration of analgesics, there is nursing care that can relieve pain,

    which the nurse can use, for example change position or give information.

    Conclusion: There are many factors that influence the nurse´s pain

    assessment, where the patient´s participation and the nurse´s attitude are

    important components. Also nursing care can ease pain.

    Abstract:

    It often means a grievous grief for relatives when a beloved person's life is coming to an end, which also means that the life you have collectively shared is coming to an end. In accordance with person-centered care, health care professionals should work for a partnership between relatives, the patient and the healthcare professional. In palliative care, relatives often take a major responsibility for the patient's care, while at the same time they are in crisis and they also have a need for support and information. Care interventions to the patient are often based on the needs that the relative´s communicated. In order to carry out a person-centered care, it is therefore important to make clear what experiences the relatives and the health professionals have of communication in palliative care.

    Aim:

    The aim was to compile studies describing how communication is experienced by relatives and healthcare professionals in palliative care.

    Methods:

    A literature review based on 15 scientific articles, all of which were collected in the CINAHL database. The articles were of qualitative and quantitative design.

    Results:

    It has positive significance for the patient and the relative´s that initially already at a breakpoint communication include relative´s. Structured communication models between related and health professionals favored the dialog between them. It was considered difficult to structure how communication shall be performed, when communication takes place at an individual level.

    Conclusions:

    There were differences of the quality of communication depending on whether the healthcare professional worked in specialized palliative care. When relative´s felt insecure and anxious, it affected the patients. Health professionals experienced differences in what the patient wanted for arrangements and what the relative´s mediated that the patient needed.
