    Abstract [en]

    Background: How the midwife treats the women impacts her experience of childbirth. Women with GDM experience the diagnosis as stressful and worrisome, time-consuming and they wish for a pregnancy without GDM. The number of studies about their childbirth experience is low. Aim: A comparison of the self-assessed childbirth experience between women with a normal pregnancy and women with a GDM-diagnosis. To identify factors that can impact on the childbirth experience. Methods: A retrospective cross-sectional study. Data was collected thru a survey, using the VAS-scale, and registerdata from MFR. A total of 391 participated, 326 with a normal pregnancy and 65 with GDM. The material was analyzed using parametric and non-parametric statistics. Result: A total of 36,1% had a negative childbirth experience. It was more common within the GDM-group (55,4%) to have a negative experience than for women with a normal pregnancy (32,2%). Factors correlating with a negative experience was, a high BMI, born outside of Sweden, primiparous and if the women were unsatisfied with the provided pain-relief. Conclusion: Women with a GDM-diagnosis has a significantly more negative childbirth experience, factors related to pregnancy and childbirth showed a correlation with a negative childbirth experience. Clinical application: The results can be used to improve healthcare for women with GDM to contribute to a more positive childbirth experience.

    Syfte; Syftet med denna litteraturöversikt var att beskriva upplevelsen av vad som skapar en meningsfull tillvaro samt hur sjuksköterskan kan stödja och främja en meningsfull tillvaro för de äldre vid särskilda boenden. Metod; Studien genomfördes som en litteraturöversikt. Artiklar publicerade mellan åren 2000-2012 söktes i databaserna CHINAL, ELITE, PubMed, SweMed samt i tidskriften Vård i Norden. Resultat; Resultatet visade på att upprätthållandet av kroppens fysiska och mentala förmågor hade stor inverkan för den upplevda hälsan. Social gemenskap, goda vårdrelationer och ett aktivt delagande i den egna vardagen ansågs även placera den äldre i ett sammanhang, vilket också grundade för meningsfullhet och välbefinnande. Konklusion; För att uppnå en god omvårdnad av de äldre krävdes sjuksköterskor med ett stort kunnande och engagemang där goda omvårdnadsrelationer skapades utifrån den enskildes livsberättelse, behov och önskningar.

    Detta är en litteraturstudie som syftar till att undersöka hur judendomen och buddhismen förhåller sig till sexualitet. Den syftar även till att undersöka vilka skillnader det finns i tolkningen av sexualiteten beroende på om forskningen sker med en mer traditionell vetenskaplig forskning eller genom en feministisk forskning. Den teori som ligger till grund för undersökningen baseras på en feministisk värdegrund som vidhåller att det inom sexualiteten finns en tydlig könsmaktsordning och genushierarki. I undersökningen presenteras ett antal texter kopplade till judendomen, buddhismen och sexualitet. Därefter analyseras de olika texterna baserat på dess forskningsinriktning för att se hur maktstrukturerna inom sexualiteten lyfts fram. Här framkommer att det inom den feministiska forskningen läggs en större fokus på att framhålla den maktkonstruktion och de könsroller som konstrueras i förhållande till religionerna. Dock framkommer även denna maktstruktur inom den mer traditionella forskningen. Slutligen sker en diskussion i förhållande till resultat och analys med fokus på den problematik och de möjligheter som kan uppstå när det skall undervisas om sexualitet i religionsämnet på gymnasieskolan. Här framkommer att det finns en problematik med att undervisa om kontroversiella frågor men att det med hjälp av en deliberativ metod finns goda möjligheter att ändå lyckas med denna undervisning.

    Detta är en kvantitativ empirisk studie som syftar till att undersöka elevers synsätt och uppfattning kring ämnesområdet sexualitet i religionskunskapsundervisningen på gymnasiet. Den syftar även till att undersöka i vilken utsträckning området sexualitet ges plats i religionsundervisningen. Den teori som ligger till grund för undersökningen är ett interkulturellt pedagogiskt perspektiv. Teorin framhåller vikten av att elever ges möjlighet att ges en inblick och förståelse för andra människors synsätt genom att samarbeta och agera tillsammans. Genom att lära sig se de delar som knyter människor samman kan även eleverna lära sig acceptera det som skiljer människor åt. Undersökningens material utgörs av enkätsvar från femtiosex gymnasielever från fem olika skolor i Sverige. Elevernas svar analyseras och tolkas för att besvara undersökningens fyra frågeställningar. I analysen fram kommer att området sexualitet lyfts fram i en ytterst begränsad utsträckning i religionsämnet. Det framkommer även att eleverna generellt sett har en positiv inställning till området sexualitet och anser detta vara av stor vikt. Slutligen sker en diskussion i förhållande till resultatet och tidigare forskning som finns i ämnet. Här framkommer att området sexualitet bör ges en större plats inom religionsämnet samt att sexualitet är ett känsligt område att undervisa kring, särskilt i förhållande till elever med en religiös positionering. Ämnet och området medför en stor utmaning för den undervisande läraren.

    Abstract [en]

    Background In person-centered care the patient and his/her experience of illness, disease, social content and close relationships is central. Person-centered care emanates from three keywords developed at University of Gothenburg Centre for Person-Centered Care (GPCC): Partnership, the patient narrative and documentation. Purpose To describe the experiences nurses have from person-centered care and how they work with partnership, the patient narrative and documentation at general units in nursing homes. Method Descriptive study conducted with a qualitative deductive approach. The study was based upon eight semi-structured interviews and data was analyzed using qualitative content analysis. Results One theme was identified: The communicative conversation used as a tool in order to perform person-centered care. Three categories were predetermined and the analysis resulted in seven subcategories: Partnership; to utilize the patient´s knowledge, to utilize the knowledge held by relatives, caregivers and the nurse, The patient narrative and Documentation; documentation of the story of the patient´s life, documentation in individualized care-plans and documentation in the system of documentation from a personal- centered perspective. Conclusion The communicative conversation was essential in order to perform person-centered care. Within this conversation, the nurse will be able to develop a relationship with the patient and from that relationship a partnership will evolve. In addition, a communicative conversation is of crucial importance in order to evolve relationships with relatives and to maintain functional cooperation. However, it turned out that the nurses experienced that they worked too far from the older person and that they did not always have time for in-depth conversations.

    Stress på jobbet har väckt stor uppmärksamhet de senaste åren. Generellt kan man säga att den psykiska ohälsan ökar. Kvinnor och lärare är överrepresenterade bland de utbrända. Syftet med undersökningen var att genom en enkätstudie ta reda på om det fanns några könsskillnader gällande mäns och kvinnors psykiska ohälsa på två gymnasier i Dalarna. Undersökningen bestod av ett frågeformulär bestående av 18 frågor. Sammanlagt 78 personer valde att delta i studien. Resultatet visade inga signifikanta könsskillnader. Man kan däremot utläsa en tendens till att kvinnorna mår sämre än männen. Då tidigare forskning pekar mot att kvinnor mår sämre än män, kan undersökningens utslag bero på för få deltagare eller på undersökningens låga svarsfrekvens.

    Abstract [en]

    In Sweden, prospective fathers are encouraged and welcome to attend prenatal visits, and pregnant women assess their partners’ involvement in prenatal care as very important. The aim of this study was to describe expectant fathers’ experiences of and involvement in prenatal care in Sweden. Data were drawn from a 1-year cohort study of 827 Swedish-speaking fathers recruited during their partners’ mid-pregnancy and followed up two months after childbirth. The participants reported that the most important issues in prenatal care were the woman’s physical and emotional wellbeing and the support she received from her midwife. However, care was identified as deficient in nearly all aspects of information, medical care, and fathers’ involvement. “Excessive” care was also reported and related to how the father was treated by the midwife, mainly in terms of attention to his emotional wellbeing. Although fathers prioritize the needs of their pregnant partners, it is important for caregivers to assess fathers’ needs and incorporate a family-oriented approach to prenatal care.

    Health workforce perspectives of barriers inhibiting the provision of quality care in Nepal and Somalia: A qualitative study2020In: Sexual and Reproductive Healthcare, ISSN 1877-5756, Vol. 23Article in journal (Refereed)
    Abstract [en]

    Objective

    In this paper settings from Nepal and Somalia are used to focus on the perspectives of healthcare providers within two fragile health systems. The objective of this study was to describe barriers inhibiting quality healthcare in Nepal and Somalia from a health workforce perspective.

    Methods

    Data were collected through 19 semi-structured interviews with healthcare providers working in healthcare facilities. Ten interviews were conducted in Nepal and nine in Somalia.

    Results

    Various structural barriers inhibiting the availability, accessibility, and acceptability of the quality care were similar in both countries. Barriers inhibiting the availability of quality care were linked to healthcare providers being overburdened with multiple concurrent jobs. Barriers inhibiting the accessibility to quality healthcare included long distances and the uncertain availability of transportation, and barriers to acceptability of quality healthcare was inhibited by a lack of respect from healthcare providers, characterised by neglect, verbal abuse, and lack of competence.

    Conclusions

    Inequality, poverty, traditional and cultural practices plus the heavy burden placed on healthcare providers are described as the underlying causes of the poor provision of quality care and the consequential shortcomings that emerge from it. In order to improve this situation adequate planning and policies that support the deployment and retention of the healthcare providers and its equitable distribution is required. Another important aspect is provision of training to equip healthcare providers with the ability to provide respectful quality care in order for the population to enjoy good standard of healthcare services.

    Abstract [en]

    Sexual and reproductive health and rights have been identified as the core areas of practice, theory and research for the midwifery profession. The midwifery profession, as defined by the International Confederation of Midwives Global Standards, is new in Bangladesh. This paper aims to give an overview of the historical process that has taken place in the past decades to develop the midwifery profession in Bangladesh. The professional development of midwives is the result of many years of collaboration and relationship building among stakeholders such as the government, non-government organizations, academia, professional associations, United Nations agencies, and donors. All are working toward achieving the common goal of preventing/ reducing maternal and child mortality through education, the development of supportive laws, policies and guidelines, and the deployment of midwives. Lessons learnt from the Bangladesh experience can provide useful guidance for other countries in Asia that are currently establishing midwifery as a profession that aims to provide safe and high quality sexual, reproductive, maternal and newborn care.

    Objective

    The aim of this paper was to identify opportunities and challenges when building a midwifery profession in Bangladesh and Nepal.

    Methods

    Data were collected through 33 semi-structured interviews with government officials, policy-makers, donors, and individuals from academia and non-government organizations with an influence in building a midwifery profession in their respective countries. Data were analyzed using content analysis.

    Findings

    The opportunities and challenges found in Bangladesh and Nepal when building a midwifery profession emerged the theme “A comprehensive collaborative approach, with a political desire, can build a midwifery profession while competing views, interest, priorities and unawareness hamper the process”. Several factors were found to facilitate the establishment of a midwifery profession in both countries. For example, global and national standards brought together midwifery professionals and stakeholders, and helped in the establishment of midwifery associations. The challenges for both countries were national commitments without a full set of supporting policy documents, lack of professional recognition, and competing views, interests and priorities.

    Conclusion and clinical application

    This study demonstrated that building a midwifery profession requires a political comprehensive collaborative approach supported by a political commitment. Through bringing professionals together in a professional association will bring a professional status. Global standards and guidelines need to be contextualized into national policies and plans where midwives are included as part of the national health workforce. This is a key for creating recognized midwives with a protected title to autonomously practice midwifery, to upholding the sexual and reproductive health and rights for women and girls.

    Abstract [en]

    BACKGROUND: With professional midwives being introduced in Bangladesh in 2013, the aim of this study was to describe midwifery students perceptions on midwives' realities in Bangladesh, based on their own experiences.

    METHOD: Data were collected through 14 focus group discussions that included a total of 67 third-year diploma midwifery students at public nursing institutes/colleges in different parts of Bangladesh. Data were analyzed deductively using an analytical framework identifying social, professional and economical barriers to the provision of quality care by midwifery personnel.

    RESULTS: The social barriers preventing midwifery quality care falls outside the parameters of Bangladeshi cultural norms that have been shaped by beliefs associated with religion, society, and gender norms. This puts midwives in a vulnerable position due to cultural prejudice. Professional barriers include heavy workloads with a shortage of staff who were not utilized to their full capacity within the health system. The reason for this was a lack of recognition in the medical hierarchy, leaving midwives with low levels of autonomy. Economical barriers were reflected by lack of supplies and hospital beds, midwives earning only low and/or irregular salaries, a lack of opportunities for recreation, and personal insecurity related to lack of housing and transportation.

    CONCLUSION: Without adequate support for midwives, to strengthen their self-confidence through education and through continuous professional and economic development, little can be achieved in terms of improving quality care of women during the period around early and late pregnancy including childbirth.The findings can be used for discussions aimed to mobilize a midwifery workforce across the continuum of care to deliver quality reproductive health care services. No matter how much adequate support is provided to midwives, to strengthen their self-confidence through education, continuous professional and economic development, addressing the social barriers is a prerequisite for provision of quality care.

    Use of simulation-based learning in midwifery education programmes is crucial. Due to midwifery educators in Bangladesh were lacking competence in using such pedagogical methods in their teaching, they were invited to participate in a simulation-based learning course. In this paper, we present a study on the perceived usefulness of this course. Semi-structured individual interviews were conducted with 17 of the 28 midwifery educators participating on the course and data were analysed using inductive content analysis. Findings showed that the simulation-based learning course for midwifery educators in Bangladesh was useful. It "builds the professional competence of midwifery educators" and "equips them with strategies to empower midwifery students". The findings show that a simulation-based learning course is of major importance in pre-service education in settings where the capacity of midwifery educators needs to be strengthened. However, without continuous in-service training, the midwives' competence will deteriorate and this in turn will threaten the quality of midwifery education and the midwifery profession. Thus, contextualized pre- and in-service simulation-based education to secure midwifery core competencies is necessary. Simultaneously implementing and evaluating pre- and in-service education programmes is the next step in the struggle to increase the quality of maternity care services.

    Abstract [en]

    OBJECTIVE: using the International Confederation of Midwives (ICM) Global Standards for Midwifery Education as a conceptual framework, the aim of this study was to explore and describe important 'must haves' for inclusion in a context-specific accreditation assessment tool in Bangladesh.

    DESIGN: A questionnaire study was conducted using a Likert rating scale and 111 closed-response single items on adherence to accreditation-related statements, ending with an open-ended question. The ICM Global Standards guided data collection, deductive content analysis and description of the quantitative results.

    SETTING: twenty-five public institutes/colleges (out of 38 in Bangladesh), covering seven out of eight geographical divisions in the country.

    PARTICIPANTS: one hundred and twenty-three nursing educators teaching the 3-year diploma midwifery education programme.

    FINDINGS: this study provides insight into the development of a context-specific accreditation assessment tool for Bangladesh. Important components to be included in this accreditation tool are presented under the following categories and domains: 'organization and administration', 'midwifery faculty', 'student body', 'curriculum content', 'resources, facilities and services' and 'assessment strategies'. The identified components were a prerequisite to ensure that midwifery students achieve the intended learning outcomes of the midwifery curriculum, and hence contribute to a strong midwifery workforce. The components further ensure well-prepared teachers and a standardized curriculum supported at policy level to enable effective deployment of professional midwives in the existing health system.

    KEY CONCLUSIONS: as part of developing an accreditation assessment tool, it is imperative to build ownership and capacity when translating the ICM Global Standards for Midwifery Education into the national context.

    IMPLICATIONS FOR PRACTICE: this initiative can be used as lessons learned from Bangladesh to develop a context-specific accreditation assessment tool in line with national priorities, supporting the development of national policies.

    användning och beskrivning av Batman samt hur upplevelsen av filmen ändras när

    man ser den utan musik. Kompositörerna till filmerna är, för

    Batman: The Movie

    (1966) Nelson Riddle, till

    Batman (1989) är det Danny Elfman och till The Dark

    Knight

    (2008) är kompositörerna Hans Zimmer och James Newton Howard. Filmerna

    analyserats från ett multimodalt perspektiv och delar in filmmusiken i filmmusikens

    berättarfunktioner framtagna av Johnny Wingstedt Scenerna har setts på flera gånger

    både med och utan musik för att få fram musikens roll och betydelse. En scen per film

    analyseras. Scenerna är valda på så sätt att de beskriver Batman som karaktär på ett bra

    sätt. Scenerna beskrivs tydligt och en analys görs innan det avslutas med en

    konklusion. Scenerna jämförs sedan med varandra för att se likheter och skillnader.

    Abstract [sv]

    Undersökningen är en uppföljning av en studie av Vasaloppet som gjordes 1992 vid Högskolan i Falun/Borlänge. Denna undersökning omfattar fem skidtävlingar (Vasaloppet, Öppetspår, Tjejvasan, Halvvasan och Kortvasan samt publiken på Vasaloppet och Kortvasan). Undersökningen har dels gjorts på plats i Mora (publikstudier)samt genom en webbaserad enkät med deltagare i de aktuella loppen. Att vasaloppsveckan 2004 attraherar ca 43.000 skidåkare avspeglas i att evenmaget vuxit från en omsättning strax under 40 miljoner kronor till ca 115 miljoner kronor drygt 10 år senare. Detta gäller de ekonomiska effekter som hamnar i de tre värdkommunerna för tvälingarna nämligen Malung, Älvdalen och Mora och i några av grannkommunerna. Vasaloppet ger också upphov till en betydande turism i form av träningsresor med övernattning. Sammantaget uppskattas att dessa förberedelser ger upphov till strax under 100.000 träningsdagar. Lejonparten av dessa äger rum i ett område med ytterändarna i Torsby, Harsa och Bruksvallarna. I stora delar bekräftar undersökningen 2004 det som framkom 1992. Allting har blivit betydeligt större men åkarnas starka intresse för motion och friluftsliv består oförändrat.

    Vasaloppsveckan: Assessing the development and impact of a major ski event in Sweden2005In: International Conference Assessing the impact of tourist events, Juan les Pins, 2005Conference paper (Refereed)
    Abstract [en]

    This paper deals with the development of a major Swedish ski event, which takes place in the first week of March every year. The main race is called Vasaloppet. It is named after a Swedish king who allegedly on his escape from the Danes in the early 1500 hundreds first travelled the 90 kilometre distance from the town of Mora to the small village of Sälen. Sälen is today the most significant alpine resort in Sweden with a total accommodation capacity of around 60,000 beds. The Vasaloppet race starts in Sälen and ends in Mora. In 1922 when the first Vasaloppet race took place the number of contestants were some 130 skiers. About 80 years later the number of races have grown to in all seven different ones and the total number of participants runs close to 45,000. In addition to this there are some 10,000 spectators coming to Mora to see Vasaloppet alone. In the year 1990 the entire competition had to be cancelled as a result of an unusually mild winter resulting in lack of snow. At that time no accurate estimates of the economic impact of the event were available and at the same time there were a great demand for estimates of the economic loss, which the region faced as a result of the event being called off. Thus, in the year 1992 a major study was undertaken by the author in order to assess the economic impact of the Vasaloppet ski race. In fact at that time the one competition from 1922 had developed into three different competitions, namely Vasaloppet, Öppet Spår and Tjejvasan. The first two are both 90 kilometres in distance but Tjejvasan, only open to female participation, runs over 30 kilometres. Then in 2004 there is a need for an updated study and now the Vasaloppet is in fact an event consisting of seven different races. The study from 2004 is based on a web survey. All participants which had a known e-mail address were contacted by e-mail and were asked to participate and were provided with a link to the website where the questionnaire was located. Foreign participants were asked to complete a questionnaire in English. The returns showed a slight bias, respondents being on average younger than non-respondents and those for which an e-mail address was missing. In order to compensate for this skewness the entire sample was weighted based on age. The response rate was fairly low, some 20% on average for the different races. A contributing factor seems to be that the questionnaire could only be reached by specifying the starting number as an entering code. Several skiers had forgotten their number and that have most likely deterred quite a few. Among the findings are that the impact in real terms appears linear and thus it has about tripled since 1992. Vasaloppet, which now is a full week of different races – Vasaloppsveckan –, clearly shows the potential attributable to recurring events. When an event is recurring it makes sense to view marketing as part of the investment for future returns and the benefits don not have to be captured in one moment, as is the case for events only taking place once in the same location. The races during the Vasaloppet week have different market areas and slightly different segments of skiers as well. Vasaloppet, the main race has grown to a considerable international presence, which runs close to 20%. The distance decay function in the market area almost starts at the border of Sweden. A significant finding of the 2004 study is that Vasaloppsveckan makes people travel away from home including overnight stay in order to get in shape for the race. It is estimated that training away from home amounts to a total of around 96,000 training days, which is perhaps slightly less in nights away from home but yet it represent a substantial value. The lion’s share of all this ski tourism is found in major resorts not to far away from Mora. Notably the resorts of Sälen, Idre and Funäsdalen are well represented. Thus if Vasaloppsveckan generates a total direct turnover of some 115 million SEK (12.8 million euro) training away from home will generate a turnover which could be estimated to be in the neighbourhood of 75% of the Vasaloppsveckan itself. The ski event also generates summer tourism in the area. The study has not made any measures of expenditures on skiing equipment etc which takes place in the home locations of the participants. Nonetheless the study shows that a well managed recurring event can turn into a significant business activity for the local economy concerned but also that an event can create considerable effects which are both trickle-down and trickle-up in nature.

    Abstract [en]

    This article investigates the relationship between technology and narrative in digital guides for tourists in Sweden. The main finding shows that narratives in the guides are based on a naïve view of history. In several instances, content is based on odd stories and speculation. Recent social trends have led to the erosion of grand narratives and the emergence of multiple views of history. New digital guides must address these challenges in order to develop tourism products that are relevant to different market segments.

    In the contemporary competition state, many peripheral regions are in decline. Tourism is increasingly considered as a savior for regions lagging behind. The EU has launched several programs with the ambition to stimulate growth and employment in peripheral areas which are used in tourism development projects. In this paper a longitudinal analysis of changes in Swedish tourism guest nights with an attempt to assess the spatial changes that have taken place in the light of this new regional tourism policy are carried out. The aim is to investigate if tourism and tourism policy contributes to level out disparities between regions. The analysis is based on statistics on Swedish guest nights, on employees in the accommodation sector and on EU funds. Although there are exceptions, the main findings indicate that the potential for creating sustainable rural tourism growth through tourism policy seems to be much less than the popular discourse suggests.

    Abstract [en]

    In this article, the development and changes in Swedish public policy relating to tourism from the 1930s to 2010 is described and interpreted from a political economy perspective. A case study, compiled from mainly secondary sources, is analyzed from a theoretical framework based on regulation theory. The purpose with this study is to increase the understanding of how the macro political economy context has influenced the policy-making in tourism in Sweden, but also to make a contribution to an area which seems to be quite neglected when it comes to research. The changes are analyzed according to the three periods denoted as pre-Fordism (mid-19th century-1930s), Fordism (1930s-1970s) and post-Fordism (1970s to present). It is observed how the general changes between these periods regarding aspects such as regulation and deregulation, and the degree of state involvement, have affected tourism policy making. The tourism policy making has changed from being insignificant, to a high degree of state involvement including planning, control and supervision, to a situation where the market rather than government regulation is considered as state of the art.

    In the contemporary so-called ‘competition state era’, many rural and peripheral regions are in decline. Tourism is increasingly viewed as being able to alleviate and rejuvenate regions that are facing economic difficulties. The European Union has launched several programmes with the goal of stimulating growth and employment in peripheral areas. These programmes are often used to support tourism development projects. In this paper, a longitudinal analysis of spatial changes in Swedish tourism is conducted. The analysis is based on statistics regarding overnight stays in Swedish commercial accommodation facilities. The aim is to investigate if tourism and tourism policy contribute to the reduction in disparities between regions. Although there are exceptions, the main findings indicate that the potential for creating sustainable rural tourism growth through tourism policy seems to be much less than the popular discourse suggests. © 2016 Informa UK Limited, trading as Taylor & Francis Group.

    Studien föranleds av den nya konkurrenslagstiftningen som påverkar gränsdragningen mellan privat och offentligt engagemang inom turismen. I studien görs ett försök att förstå den historiska utvecklingen utifrån institutionell teori. Att över tid följa hur turismen från att ha legat utanför samhällets intressesfär till att i allt högre grad inkluderas i och bli föremål för samhällsåtaganden till att i betydande utsträckning åter överflyttas till den privata sfären blir enligt författarnas mening begripligare om förändringsförloppet förstås i den referensram som den institutionella teorin erbjuder. Den pendelrörelse som på detta sätt kan urskiljas beträffande gränsdragningen mellan privat och offentligt är också en del i hur man skall förstå hur synen på konkurrensbetingelser inte är statisk utan i hög grad konstruerad och framförhandlad från tid till annan. Men det som gälller tid tycks också ha en geografisk dimension där vad som anses tillåtet i en del av landet inte uppfattas så på ett annat ställe. Det som uppfattas som ett juridiskt spörsmål i förstone vilar i realiteten i en komplex kontext med ingredienser av historia, ekonomi och geografi.

    Background:

    Research suggests that care based on a person-centered basis

    contributes to a higher quality of care, and has a positive impact on the patients and

    the healthcare profession. At the same time, studies point out that health care is facing

    a variety of challenges in order to introduce a person-centered approach into daily

    care.

    Aim:

    To describe which barriers prevent the nurse to work person-centered in the

    healthcare meeting with the patient.

    Methods:

    The study was conducted as a literature review, the result is based on 16

    scientific articles.

    Results:

    Several factors that may constitute difficulties for the nurse to work personcentered

    in the healthcare meeting with the patient were identified and generated four

    categories:

    Collaboration and Teamwork, Attitude and Culture, Time and Resources,

    and

    Communication and The patient Narrative. The result indicates cooperation

    difficulties between different health care professions, contradictory attitudes and

    routines that not lie on a scientific basis. Lack of educational opportunities and

    knowledge about what person centered care means and how a person-centered

    approach should be applied in clinical practice. High workload and lack of time

    affected the nurse's ability to establish a relationship with the patient, and contributed

    to stress and feelings of inadequacy.

    Conclusion:

    Summary the literature study indicates that there are several barriers that

    make it difficult for the nurse to work person-centered in the meeting with the patient.

    In order for a person-centered approach to be practiced, active work is required

    individually and collectively at different levels within an organization to become part

    of the daily practice.

    Syftet: Syftet med denna litteraturöversikt var att beskriva föräldrars upplevelse av stöd från hälso- och sjukvårdspersonalen då deras barn diagnosticerats med cancer. Metod: Examensarbetet genomfördes som en litteraturöversikt. Vetenskapliga artiklar söktes i Högskolan Dalarnas biblioteks prenumererade databaser CINAHL och SAGE Journal Online. Av de 12 vetenskapliga artiklarna som användes var fem av kvalitativ ansats, tre var av kvantitativ ansats och fyra var av mixad metod. Resultat: Resultatet visade att föräldrar till cancerdiagnosticerade barn behövde olika typer av stöd, så som känslomässigt, psykologiskt, socialt samt avlastning. Samtalsgrupper behövdes för att föräldrarna skulle få träffa andra i liknande situationer och utbyta erfarenheter. Slutsats: Hälso- och sjukvårdspersonalen bör tänka på hur kommunikation används och information ges. Varje förälder ses som unik och bör därför bemötas individuellt. Föräldrarna behövde stöd i olika former för att kunna bearbeta sitt barns cancerdiagnos. Hälso- och sjukvårdspersonalen bör sträva efter en fungerande relation till barnet och föräldrarna för att skapa förtroende och en förutsättning för bästa möjliga vård.

    BACKGROUND: There is limited knowledge about leisure time physical activity and the body mass index (BMI) as prognostic factors for recovery from persistent back pain. The aim of this study was to assess the influence of leisure time physical activity and BMI on recovery from persistent back pain among men and women in a general population.

    METHODS: The study population (n=1836) in this longitudinal cohort study consisted of participants reporting persistent back pain in the baseline questionnaire in 2002-2003. Data on leisure time physical activity, BMI and potential confounders were also collected at baseline. Information on recovery from persistent back pain (no back pain periods ≥ 7 days during the last 5 years) was obtained from the follow-up questionnaire in 2007. Log-binomial models were applied to calculate Risk Ratios with 95 percent Confidence Intervals (CI) comparing physically active and normal weight groups versus sedentary and overweight groups.

    RESULTS: Compared to a sedentary leisure time, all measured levels of leisure time physical activity were associated with a greater chance of recovery from persistent back pain among women. The adjusted Risk Ratios was 1.46 (95% CI: 1.06, 2.01) for low leisure time physical activity, 1.51 (95% CI: 1.02, 2.23) for moderate leisure time physical activity, and 1.67 (95% CI: 1.08, 2.58) for high leisure time physical activity. There were no indications that leisure time physical activity influenced recovery among men, or that BMI was associated with recovery from persistent back pain either among men or among women.

    CONCLUSIONS: Regular leisure time physical activity seems to improve recovery from persistent back pain among women.

    OBJECTIVES: To study the influence of healthy lifestyle behaviour on the prognosis of occasional low back pain among men and women in a general population.

    DESIGN: Cohort study with a 4-year follow-up.

    SETTINGS: General population in Stockholm County, Sweden.

    PARTICIPANTS: The study sample comprised 3938 men and 5056 women aged 18-84 from the Stockholm Public Health Cohort reporting occasional low back pain in the baseline questionnaire 2006.

    MEASURES: Lifestyle factors and potential confounders were assessed at baseline. The lifestyle factors smoking habits, alcohol consumption, leisure physical activity and consumption of fruit and vegetables were dichotomised using recommendations for a health-enhancing lifestyle and combined to form the exposure variable 'healthy lifestyle behaviour'. The exposure was categorised into five levels according to the number of healthy lifestyle factors met. The follow-up questionnaire in 2010 gave information about the outcome, long duration troublesome low back pain. Crude and adjusted binomial regression models were applied to estimate the association between the exposure and the outcome analysing men and women separately.

    RESULTS: The risk of developing long duration troublesome low back pain among women with occasional low back pain decreased with increasing healthy lifestyle behaviour (trend test: p=0.006). 21% (28/131) among women with no healthy lifestyle factor (reference) experienced the outcome compared to 9% (36/420) among women with all four factors. Compared to the reference group, the risk was reduced by 35% (RR 0.65, 95% CI 0.44 to 0.96) for women with one healthy lifestyle factor and 52% (RR 0.48, 95% CI 0.31 to 0.77) for women with all four healthy lifestyle factors. There were no clear associations found among men.

    CONCLUSIONS: Healthy lifestyle behaviour seems to decrease the risk of developing long duration troublesome low back pain among women with occasional low back pain and may be recommended to improve the prognosis.
