Abstract [sv]

Bakgrund: Hjärtsvikt är ett globalt folkhälsoproblem som drabbar cirka 26 miljoner människor. Att leva med hjärtsvikt innebär en livsomvälvande förändring och försämrad hälsa för patienten relaterat till hjärtats försämrade pumpförmåga. Ett nytt liv med sjukhusbesök, behandlingar, medicinering och sjukhusinläggningar, skapar i många fall oro hos patienten för deras hälsa och framtid och i förlängningen även ångest och depressioner. Det är inte bara patienten som drabbas utan även dennes anhörige och familj är i många fall högst involverade i patientens vardag och egenvård. Syfte: Syftet är att beskriva patienters erfarenheter att följa egenvårdsråd vid hjärtsvikt. Metod: En litteraturöversikt för att sammanställa aktuell forskning utfördes. Resultat: Att bli diagnostiserad med hjärtsvikt innebar en stor omställning och påverkan på vardagen. Förmåga till acceptans, relationen till familj och anhöriga, tidigare erfarenheter, psykiska och fysiska begränsningar var faktorer som påverkade patientens följsamhet till egenvård. Att följa egenvårdsråden påverkade i stor utsträckning vardagen och livet i olika grad hos patienterna med hjärtsvikt. Det framkom att patienterna måste acceptera sitt hälsotillstånd och övervinna de fysiska och psykiska hindren med stöd utifrån det individuella behovet för att klara av att utföra egenvårdsråden som givits. Det uppmärksammades även att en del patienter saknade tillräckligt med stöd och information. Med rätt stöd och utbildning från hälso- och sjukvården ökade följsamhet till egenvård och därmed underlättades vardagen och färre återinläggningar kunde ses. Konklusion: Litteraturöversiktens resultat visade varierande faktorer och erfarenheter kring följsamhet till egenvård. Kunskap, acceptans och förståelse kring sjukdomen var betydande för att patienten skulle utföra egenvården. Negativa faktorer för utförandet av egenvård var psykisk ohälsa och fysiska begränsningar, vilka även hade påverkan gentemot varandra och minskade följsamheten till egenvård. Stöd och hjälp från anhöriga i samråd med sjukvården var en viktig grund för att hantera sin egenvård.

Abstract [en]

Background: Heart failure is a global public health problem which affects approximately 26 million people. Living with heart failure means a life-changing event and impaired health for the patient related to the hearts decreased ejection fraction. In many cases, a new life with hospital visits, treatments, medication and hospitalizations creates concern for the patient about their health and future and, in the long run, also leads to anxiety and depression. It is not only the patient who is affected, but also his or her family and relatives are often involved in the patient's everyday life and self-care.  Objective: To describe patients' experiences of adherence to self-care advice in heart failure.  Method: A literature review to compile current research was done.  Results: Being diagnosed with heart failure meant a major change and impact on everyday life. Acceptability, the relationship with family and relatives, past experiences, psychological and physical limitations were factors that affected the patient's adherence to self-care. Following self-care advice affected everyday life to a varying degree in patients with heart failure. It emerged that patients must accept their state of health and overcome the physical and mental barriers, with support based on the individual need, to cope with the self-care advice given. It was also noted that some patients lacked enough support and information. With the right support and education from the healthcare system, compliance to self-care increased and thus made everyday life easier and fewer readmissions could be seen.  Conclusion: The results of the literature review showed varying factors and experiences regarding adherence to self-care. Knowledge, acceptance and understanding of the disease were a significant basis for self-care. Negative factors for adherence to self-care were mental- and physical limitations, which also had an impact on each other and reduced the adherence to self-care. Support and assistance from relatives in consultation with the healthcare system was an important basis for managing their self-care.
