Abstract [sv]

Bakgrund: Hypertoni är en riskfaktor för att utveckla andra kardiovaskulärasjukdomstillstånd som stroke, hjärtsvikt, kranskärlsjukdom och ateroskleros, vilketkan leda till för tidig död. En stor del av behandlingen vid hypertoni består avlivsstilsförändringar. Trots att det finns mycket kunskap om livsstilsförändringaroch dess effekt på hypertoni så är följsamheten låg och förekomsten av hypertonifortfarande hög.Syfte: Syftet med denna litteraturöversikt var att belysa aspekter som påverkarpatienters följsamhet till rekommenderade livsstilsförändringar vid hypertoni.Metod: En litteraturöversikt med datainsamling av vetenskapliga artiklar frånCINAHL och PubMed. Resultatet grundas på analys av 15 vetenskapliga artiklar.Resultat: Resultatet visade flera olika aspekter som påverkade följsamheten tillrekommenderade livsstilsförändringar vid hypertoni. Det fanns flera personligaaspekter som påverkade följsamheten negativt, till exempel brist på viljestyrka,självförtroende samt disciplin. Patientens livssituation som låg utbildning, låginkomst, arbetslöshet samt tidsbrist i vardagen visades ha negativ effekt påföljsamheten. Patienterna hade otillräcklig kunskap om sjukdomens orsaker ochbehandling samt hur livsstilen påverkade blodtrycket, otillräcklig kunskap leddetill bristande följsamhet. Kommunikationen mellan patient och vårdpersonal var enviktig faktor för att möjliggöra för patienten att genomföra livsstilsförändringar.Slutsats: Denna litteraturöversikt visar att olika aspekter påverkar patientersföljsamhet till rekommenderade livsstilsförändringar hos patienter med hypertoni,vilket kan göra det svårt att följa dessa. Att tillämpa generaliseradelivsstilsförändringar är svårt eftersom följsamheten till dessa influeras av mångaaspekter. Att individuellt anpassa livsstilsförändringarna skulle kunna möjliggöraför att få mer fördelaktigt behandlingsresultat och därmed förbättra hälsa på bådeindivid- och samhällsnivå.

Abstract [en]

Background: Hypertension is a risk factor to develop cardiovascular disease suchas stroke, heart failure, coronary heart disease and atherosclerosis, which canincrease the risk of early death. A big part of the treatment of hypertension islifestyle changes. Despite the high amount of knowledge about lifestyle changesand their effect on hypertension, the adherence is low, and the prevalence ofhypertension is high.Aim: The aim with this literature review was to illuminate aspects that affectadherence to recommended lifestyle changes in patients with hypertension.Method: A literature review. Data was collected from CINAHL and PubMed. Theresult is based on an analysis of 15 articles.Results: The result showed several aspects that affects adherence to recommendedlifestyle changes in hypertension. Lack of willpower, confidence, discipline, loweducation, low income, unemployment and lack of time in everyday life had anegative effect on the adherence. The patients had lack of knowledge about thedisease and its treatment and how the blood pressure was affected by the lifestylechanges which led to deficient adherence. Communication between patient andhealth care workers was an important factor for enabling to perform lifestylechanges.Conclusion: Several aspects affect adherence to recommended lifestyle changes inpatients with hypertension, which could make them difficult to follow. Therefore,generalized lifestyle changes can be difficult to imply. To individualize thelifestyle changes could enable to more favorably results in treatment and thereforeenhance health in both individual and society.
