Core Exercise and Diastasis Recti Abdominis : What effect does core training have on health-related quality of life in women with diastasis recti? A structured literature review (English)
Abstract [en]

Background: Diastasis Recti Abdominis (DRA) is a separation of the abdominal muscles that affects two out of three women (66%) in the third trimester. Persistent abdominal rectus diastasis has been noted to lead to poorer quality of life (HRQoL) for mothers. Physiotherapy is prioritized as a treatment modality where core training shows a modest clinical effect on inter-recti distance. However, there is a lack of reviews on how women's HRQoL is affected by exercise therapy.

Objective: To conduct a structured literature review mapping randomized studies comparing core training with other or no training regarding its effect on HRQoL in women with Diastasis Recti Abdominis (DRA) postpartum. Additionally, the aim was to delineate the interventions used regarding the dosage of core training.

Method: Data collection was conducted through PubMed, Web of Science, and Cinahl on February 27, 2024. The methodological and ethical quality of relevant articles was reviewed. Risk of bias was assessed. Data were extracted and compiled. The reliability of the result was assessed using GRADE.

Results: Following a literature search yielding 141 hits and the review of relevant articles, four studies were included. These studies utilized different methods of core training over periods of up to 12 weeks. The results showed heterogeneous effects on HRQoL, measured with the Oswestry Disability Index (ODI), Pelvic Floor Disability Inventory (PFDI), and Inventory of Functional Status after Childbirth (IFSAC). The risk of bias was assessed to below to moderate. Only one study reported a sufficiently large and statistically significant improvement in HRQoL. In summary, the results demonstrated heterogeneity and moderate reliability regarding the effect of core training on HRQoL, necessitating further research to confirm these findings.

Conclusion: Further studies with more robust methodology, larger samples, well-defined training protocols, and with HRQoL as the primary outcome measure are necessary to better understand the intervention's effect on HRQoL and the significance of training dosage in this regard.

Bakgrund: Diastasis recti abdominis (DRA) är en separation av magmuskler som drabbar två av tre kvinnor (66%) i tredje trimestern. Kvarstående abdominell rektus diastas har noterats leda till sämre HRQoL för mödrar. Fysioterapi prioriteras som behandlingsform där bålträning visar en liten klinisk effekt på diastasens avstånd. Dock saknas översikter om hur kvinnornas HRQoL påverkas av träningsbehandling.

Syfte: Att via en strukturerad litteraturöversikt kartlägga randomiserade studier som jämfört bålträning med annan eller ingen träning avseende dess effekt på HRQoL hos kvinnor med DRA postpartum. Därtill var syftet att kartlägga de interventioner som använts avseende dosering av bålträning.

Metod: Datainsamlingen genomfördes genom PubMed, Web of Science och Cinahl 2024-02-27. Artiklarnas metodologiska och etiska kvalitet granskades. Risken för bias bedömdes. Data extraherades och sammanställdes. Tillförlitligheten till resultatet bedömdes genom GRADE.

Resultat: Efter en litteratursökning och 141 sökträffar och granskning av relevanta artiklar inkluderades fyra studier i analysen. Dessa studier använde olika metoder för bålträning under perioder upp till 12 veckor. Resultaten visade heterogena effekter på HRQoL, mätt med Oswestry Disability Index (ODI), Pelvic Floor Disability Inventory (PFDI) och Inventory of Functional Status after Childbirth (IFSAC). Endast en studie rapporterade en tillräckligt stor och statistiskt signifikant förbättring av HRQoL. Sammanfattningsvis visade resultatet heterogenitet och måttlig tillförlitlighet avseende effekten av bålträning på HRQoL, med behov av ytterligare forskning för att bekräfta dessa fynd.

Slutsats: Ytterligare studier med robustare metodik, större urval, väldefinierade träningsprotokoll samt med HRQoL som primärt utfallsmått är nödvändiga för att bättre förstå interventionens effekt på HRQoL samt träningsdoseringens betydelse i detta.
