Long-term Effects of Exercise in Reducing Pain in Knee Osteoarthritis (English)
Abstract [sv]

Bakgrund: Knäartros är en vanlig form av artros och smärta är det vanligaste symptomet. Tidigare översiktsstudier har funnit att fysisk träning kan leda till minskad smärtintensitet omgående efter avslutad intervention, men kunskapsunderlaget är mer begränsat gällande hur väl träningseffekter kvarstår efter avslutad intervention.

Syfte: Syftet med detta examensarbete var att sammanställa aktuellt vetenskapligt underlag för hur landbaserad träning, i jämförelse med ingen träning eller annan behandling som inte består av fysisk träning, inverkar på smärtintensitet hos personer med knäartros, minst tre månader efter avslutad träningsintervention.

Metod: Sökning skedde i databaserna PubMed, CINAHL, PEDro och SwePub efter randomiserade studier som publicerats från och med år 2005. För att inkludera studier granskades samtliga studiers abstrakt, därefter skedde granskning av kvarvarande fulltextartiklar innan slutlig inkludering skedde. De inkluderade studierna kvalitetsgranskades med PEDro, varefter deras resultat sammanställdes. Slutligen bedömdes resultatens tillförlitlighet med GRADE.

Resultat: Totalt importerades 2994 artiklar varav fyra studier inkluderades. Samtliga studier bedömdes enligt PEDdros granskningsmall vara av god kvalitet. Två inkluderade studier fann signifikant lägre smärtintensitet i interventionsgrupp, jämfört med kontrollgrupp. De två övriga studierna fann ingen skillnad mellan kontrollgrupp och träningsgrupp (p≥0.26). Av de studier som fann signifikant skillnad utvärderades i den ena smärtintensitet med WOMAC Pain (0-20,högre=mer smärta) och fann att kontrollgrupp skattade smärta två poäng högre än interventionsgrupp (p=0.001). Den andra studien utvärderade smärta med KOOSPain (0-100, högre=mindre smärta) och fann att träningsgruppen (KOOS Pain=63) skattade smärtintensitet signifikant (95% konfidensintervall:0.45, 1.28) lägre än kontrollgruppen (KOOS pain=46). Denna skillnad var av stor effekt (Cohens d=0.8). Den sammanvägda tillförlitligheten till resultatet bedömdes vara måttlig (+++) enligt GRADE-bedömning.

Konklusion: Resultatet påvisar varierande resultat av hur landbaserad träning inverkar på smärtintensitet minst tre månader efter avslutad intervention. Detta i kombination med det begränsade underlaget gör att det vetenskapliga underlaget i nuläget bedöms vara otillräckligt.

Abstract [en]

Background: Knee osteoarthritis is a common form of arthritis, with pain being the most frequent symptom. Previous review studies have found that physical exercise can lead to reduced pain intensity immediately after the intervention, but the evidence is more limited regarding how well the effects of exercise persist after the intervention ends.

Aim: The aim of this thesis was to compile current scientific evidence on how land-based exercise, compared to no exercise or other non-exercise treatments, affects pain intensity in individuals with knee osteoarthritis at least three months after the end of the training intervention.

Method: Searches were conducted in the databases PubMed, CINAHL, PEDro, and SwePub for randomized studies published from 2005 onwards. To include studies, abstracts of all studies were reviewed, followed by a review of the remaining full-text articles before final inclusion. The included studies were quality assessed using PEDro, and their results were compiled. Finally, the reliability of the results was evaluated using GRADE.

Results: A total of 2994 articles were imported, of which four studies were included. All studies were assessed as being of good quality according to thePEDro review template. Two included studies found significantly lower pain intensity in the intervention group compared to the control group. The other two studies found no difference between the control group and the exercise group (p≥0.26). Of the studies that found a significant difference, one evaluated pain intensity using WOMAC Pain (0-20, higher=more pain) and found that the controlgroup rated pain two points higher than the intervention group (p=0.001). The other study evaluated pain using KOOS Pain (0-100, higher=less pain) and found that the exercise group (KOOS Pain=63) rated pain intensity significantly (95% confidence interval: 0.45, 1.28) lower than the control group (KOOS Pain=46). This difference was of large effect (Cohen's d= 0.8). The overall reliability of the results was assessed as moderate (+++) according to the GRADE assessment.

Conclusion: The results show varying outcomes on how land-based exercise affects pain intensity at least three months after the end of the intervention. This, combined with the limited evidence, means that the current scientific evidence is considered insufficient.
