Bakgrund: Upprätt position under födandet definieras som huksittande, sitta på förlossningspall, knästående, fyrfota, sittande i sängen och stående. Barnmorskans ansvar är att främja upprätta positioner under födandet då det medför en rad fördelar för det fysiologiska födandet. Prevalensen för upprätta positioner hos födande varierar ur ett globalt perspektiv. Syfte: Undersöka hur födande upplever en upprätt positionering under födandet och vilka erfarenheter barnmorskan har av att bistå födande som har upprätt position. Metod: Kvalitativ metasyntes med metaetnografi som analysmetod. Artikelsökning utfördes i databaserna CINAHL och PubMed. Efter kvalitetsbedömning inkluderades 16 vetenskapliga artiklar. Resultat: En övergripande syntes identifierades, I stillhet och storm genom födandet: födandet var en process som växlade snabbt från lugn och stillhet till oro och stress, det var en oförutsägbar resa för både födande och barnmorska. Fem underteman framträdde, Färdas med eka eller skepp: barnmorskor beskrev hur möjligheten att bistå födande i upprätta positioner var beroende av vilka förutsättningar som rådde på arbetsplatsen. Två kaptener vid rodret: Barnmorskor hade skilda åsikter kring vem som skulle bestämma över positioneringen. Födande beskrev missnöje över att bli för styrda när barnmorskan tog kommandot, medan födande som bemöttes med respekt upplevde större välbefinnande. Navigera genom vågor: Födande betonade vikten av att vara fri och röra sig, det gav en känsla av att vara aktiv och ha makt över den egna kroppen. En ökad känsla av delaktighet och kontroll beskrevs när födande var i upprätt position. Följa ljuset av en inre fyr: Det visade sig vara betydelsefullt för såväl födande som barnmorskor att vara lyhörda och följsamma till sin intuition. Trygg hamn: Upprätta positioner främjade högre närvaro av barnmorskan, vilket utgjorde en central del i relationsskapandet. De uttryckte vikten av god kommunikation med respekt för den födande. Slutsats: Resultatet visar att upprätta positioner bidrog till att födande upplevde känslor av frihet och kontroll, det främjade deras möjlighet att följa sin intuition. Relationen med barnmorskan förbättrades när födande var upprätta och intog en mer aktiv roll, de önskade vara delaktiga i beslut kring positionering. Resultat visar att det var av stor vikt att barnmorskor gavs förutsättningar till att värna födandes autonomi och möjligheter att vara upprätta under födandeprocessen. Klinisk tillämpbarhet: Ökad medvetenhet hos barnmorskor kring upprätta positioners inverkan under födandet kan berika såväl födandeupplevelsen och det fysiologiska födandet, som mötet mellan vårdpersonal, gravida och födande. Tidiga utbildningsinsatser och samtal redan i mödravården kring födandepositioners betydelse skulle kunna vara till god hjälp för den födande och även för barnmorskan på förlossningsavdelningen.

Background: Upright position during labour is defined as squatting, birthing stool, kneeling, quadruped, sitting in bed and standing. The midwife's responsibility is to promote upright positions during childbirth as it brings several benefits to the physiological birth. The prevalence of upright positions varies from a global perspective. Aim: The aim is to investigate how birthing women experience an upright positioning during childbirth and what experiences the midwife has of assisting birthing women who have an upright position. Method: Qualitative meta synthesis with meta ethnography as an analysis method. Article searches were performed in the CINAHL and PubMed databases. After quality assessment, 16 scientific articles were included. Results: An overarching synthesis was identified, In stillness and storm through childbirth: childbirth was a process that changed quickly from calm and stillness to worry and stress, it was an unpredictable journey for both  birthing and midwifery. Five sub-themes emerged, Travelling with a spoke or ship: Midwives described how the possibility of assisting birthing women in upright positions depended on the conditions prevailing in the workplace. Two captains at the helm: Midwives had different opinions about who should decide on the positioning. Birthing women described dissatisfaction with being too controlled when the midwife took charge, while birthing women who were treated with respect experienced greater well-being. Navigating through waves: Giving birth emphasized the importance of being free and moving, it gave a feeling of being active and having power over one's own body. An increased sense of participation and control was described when the birthing woman was in an upright position. Follow the light of an inner lighthouse: It turned out to be important for both birthing and midwives to be responsive and compliant with their intuition. Safe haven: Upright positions promoted higher presence of the midwife, which was a central part of relationship building. They expressed the importance of good communication with respect for the birthing woman. Conclusion: The results show that upright positions contributed to birthing women experiencing feelings of freedom and control, it promoted to follow their intuition. The relationship with the midwife was facilitated when birthing women were upright and took a more active role in the process, they wanted to be involved in decisions about positioning during their birth. It was of great importance that midwives were given the conditions to safeguard the autonomy of birthing women and the opportunities to be upright during the birthing process. Clinical applicability: Increased awareness among midwives about the impact of upright positions during childbirth can enrich both the birth experience and the physiological birth, as well as the meeting between healthcare professionals, pregnant women and birthing women. Early educational efforts and conversations already in maternity care about the importance of birthing positions could be of good help to the birthing woman and also to the midwife in the maternity ward.
