Bakgrund: Ansvaret för patienter i primärvården delas av två verksamheter, vårdcentralen och hemsjukvården, och de har olika huvudmän – region och kommun. I framtiden kommer färre personal arbeta inom vården samtidigt som antalet äldre och multisjuka med komplexa vårdbehov ökar. Resurserna måste fördelas effektivt för att en säker vård ska kunna bedrivas. En omställning som innebär att skifta fokus från organisation och organisatoriska gränser till patientens behov pågår, God och nära vård. Samverkan inom primärvården beskrivs som avgörande för omställningen. Distriktssköterskor och sjuksköterskor i organisationerna har en viktig roll i omställningen genom samverkan med varandra. Samverkan i team kan skapa en synergistisk effekt för patientens vård. Syfte: Syftet med studien var att beskriva distriktssköterskor och sjuksköterskors erfarenheter av samverkan mellan vårdcentral och hemsjukvård. Metod: Studien genomfördes empiriskt som en kvalitativ intervjustudie. Tolv sjuksköterskor och distriktssköterskor deltog i studien. Materialet analyserades med en kvalitativ innehållsanalys, med induktiv ansats enligt Granheim och Lundman (2004). Resultat: Resultatet beskrivs i två huvudkategorier. Organisation beskriver kunskapen om varandras verksamheter och vilka organisatoriska faktorer som påverkar samverkan.Sjuksköterskans yrkesutövning beskriver erfarenheter av hur sjuksköterskorna utför arbetet kopplat till patientens vård. Slutsats: Samverkan mellan vårdcentral och hemsjukvård är bristfällig. Gemensam planering av patientens vård genomförs sällan och kommunikation med fax bidrar till att dialog uteblir. Om det fanns en fast kontakt mellan vårdcentral och hemsjukvård skulle samverkan troligtvis fungera bättre.

Background: The responsibility for patients within primary care in Sweden oscillates between two organizations, the Community Health Centers and the Home Health Services. Demographics in Sweden shows a growing older population with multimorbidity and complex care needs, while the number of health care staff is decreasing. An ongoing change in the healthcare system with the purpose to move focus from organizational boundaries to the patients’ needs is a part of the solution for allocating resources and making a more efficient care. Cooperative behaviour is described as a key factor for this change to be successful and can lead to a synergistic effect for the patients care. Aim: The purpose of the study was to describe district nurses' and nurses' experiences of collaboration between Community Health Centers and Home Health Services. Method: The study was conducted empirically as a qualitative interview study. Twelve nurses’ and district nurses’ participated in the study. The material was analysed by qualitative content analysis with an inductive approach according to Granheim and Lundman (2023). Results: The results are described in two main categories. Organization describes experiences regarding the perception of each other's organization and organizational factors that affect the collaboration. The nurse's professional practice describes experiences of how the nurses perform their work linked to the patient’s care. Conclusions: Collaboration between the Community Health Center and Home Health Services is perceived as lacking. There is no joint planning for the patients care and communication with fax machine is outdated and contribute to less cooperative behaviour. If there was a steady contact between the two organizations, collaboration would probably work better.
