Bakgrund Tydlig och strukturerad kommunikation är en förutsättning för säker vård, där brister kan leda till informationsförlust och vårdskador. Trots kommunikationens centrala roll i det dagliga arbetet ges begränsat utrymme för praktisk träning under såväl utbildningen som fortbildning. Arbetets teoretiska referensram utgörs av kommunikationsteoretikern Berlos SMCR-modell.

Syfte Att undersöka kommunikationsfaktorer som påverkar överrapportering och patientsäkerhet vid överlämningar mellan vårdpersonal inom slutenvården.

Metod Arbetet utfördes som en strukturerad litteraturöversikt med inslag av systematisk metodologi. Underlaget utgjordes av 20 artiklar från databaserna CINAHL och PubMed, dessa var av kvalitativ (n=14), kvantitativ (n=1) och mixad metod (n=5).

Resultat Resultatet visar att överrapportering påverkas av flera samverkande kommunikationsfaktorer. Tre huvudkategorier identifierades – personliga, sociala och organisatoriska – som tillsammans formar budskap och förutsättningar, belyser vårdkommunikationens komplexitet och påverkar möjligheten till en säker och strukturerad överlämning av patientinformation.

Slutsats Överrapportering inom slutenvården utgörs av ett komplext samspel mellan kommunikationsfaktorer som på olika sätt påverkar patientsäkerheten. Vikten av säker och effektiv kommunikation behöver synliggöras och tränas på, där utfallet är ett gemensamt ansvar mellan individen, vårdteamet och organisationen. 

Background Clear and structured communication is essential for safe care and communication gaps can lead to information loss and patient harm. Despite its central role in the daily work of healthcare teams, practical communication training receives limited attention in both education and professional development. The study’s theoretical framework is based on the communication theorist Berlo’s SMCR model.

Aim To examine communication factors that affect the handoff report and patient safety during handovers between healthcare professionals in inpatient care.

Method A structured literature review with elements of systematic methodology. 20 articles from CINAHL and PubMed form the basis of the literature review, these were qualitative (n=14), quantitative (n=1) and mixed methods (n=5).

Results The results show that handovers are influenced by several interacting communication factors. The three main categories of personal, social and organisational factors were identified. They shape both content and conditions, highlight the complexity of healthcare communication and affect the ability to implement a safe and structured handover of patient information.

Conclusions Handover in inpatient care is a complex interaction of communication factors that, in various ways, affect patient safety. The importance of safe and effective communication needs to be highlighted and practiced, where the outcome is a shared responsibility between the individual, the care team and the organisation.
