Abstract [sv]

Bakgrund Sjuksköterskor arbetar i en komplex vårdmiljö där etiska beslut är en del av det dagliga arbetet. När de inte kan agera enligt sina etiska värderingar kan moralisk stress uppstå, vilket kan påverka både välbefinnandet och vårdkvaliteten negativt.

Syfte Att belysa sjuksköterskors upplevelser av moralisk stress

Metod Studien gjordes som en strukturerad litteraturöversikt med inslag av systematisk metodologi. Resultatet grundas på 15 vetenskapliga artiklar. Studierna var kvalitativa studier från databaserna CINAHL och PubMed.

Resultat Två huvudkategorier som tar upp orsaker till moralisk stress framkom: Arbetsmiljö och patientens lidande. En tredje huvudkategori belyser konsekvenser som moralisk stress kan leda till. Resultatet visar att många olika situationer kan bidra till uppkomst av moralisk stress och att moralisk stress kan ge påverkan både på sjuksköterskans mående och på synen av sjuksköterskerollen.

Slutsats När sjuksköterskor av olika orsaker hindras från att agera i enlighet med sina etiska värderingar, uppstår en rad negativa känslor som har visat sig kunna få betydande konsekvenser för sjuksköterskan både fysiskt, psykiskt och socialt. Moralisk stress kan i längden även leda till känslomässig immunisering och förlorad sjuksköterske-identitet.

Abstract [en]

Background Nurses work in a complex work environment where ethical decisions is a part of their daily work. When they are unable to act according to their ethical values, moral distress can arise which negatively affects both well-being and the quality of care.

Aim To highlight nurses' experiences of moral distress

Method The study was conducted as a structured literature review with elements of systematic methodology. The result is based on 15 scientific articles. The studies were qualitative original studies sourced from the databases CINAHL and PubMed.

Results Two main categories were identified as causes of moral distress: Work environment and the patient`s suffering. A third main category highlights the consequences that moral distress can lead to. The result shows that different situations can lead to the emergence of moral distress and that moral distress can affect both the nurses' well-being and the perception of the nursing role.

Conclusions When nurses are prevented from acting accordance with their ethical values, a range of negative emotions arise, which have been shown to have significant consequences for nurses both physically, psychologically and socially. Moral distress can also lead to emotional immunzation and a loss of nurse identity.
