Abstract [sv]

Bakgrund Ländryggsmärta är ett vanligt, återkommande och samhällsekonomiskt kostsamt tillstånd. Egenvård och patientutbildning är centrala delar av egenvård inom MDT, där målet är ge patienten verktyg att självständigt hantera sina besvär. En kunskapslucka föreligger dock i förhållande till patientens upplevelse av möjliggörare och hinder och hur dessa påverkar följsamheten för egenvård.

Syfte Syftet med studien har varit att undersöka patienters upplevelser av egenvård inom MDT. Frågeställningar som avsågs besvaras var vilka faktorer som patienter klassificerade med derangement upplever som möjliggörare respektive hinder för att följa egenvård givna av fysioterapeuter med grundexamen inom MDT.

Metod Studien genomfördes som en kvalitativ intervjustudie med semistrukturerade intervjuer som sedan analyserades med en manifest innehållsanalys.

Resultat Tre kategorier utkristalliserade sig under analysen. Under kategorin Möjliggörare via fysioterapeutens kompetens och bemötande rapporterade respondenterna en stark terapeutisk allians där fysioterapeutens analys och förklaring bidrog till att skapa förtroende. Under kategorin Möjliggörare via patientens kunskap och delaktighet rapporterade respondenterna att de tillägnade sig kompetens och upplevde medbestämmande i utformandet av egenvården. Vidare bidrog enkelhet och snabba resultat till förmågan att hantera pågående episod samt recidiv. Under kategorin Patientens upplevelse av hinder förföljsamhet till egenvård. beskrivs bland annat kontextuella faktorer, bristande förståelse,upplevd hög träningsdos och bristande stöd för minnet.

Slutsats Studien identifierar upplevda möjliggörare och hinder rörande följsamhet för egenvård enligt MDT. Även om resultatet i studien baseras på ett lågt antal deltagare, kan den utgöra ett första bidrag för att utveckla följsamheten när råd om egenvård ges till patienter i ett MDT-sammanhang.

Abstract [en]

Background Low back pain is a common, recurrent, and socioeconomically costly condition. Self-care and patient education are central aspects of self-management within MDT, where the goal is to provide the patient with tools to independently manage their complaints. However, there is a knowledge gap regarding the patient’s experience of enablers and barriers, and how these influence adherence to self-care.

Purpose The purpose of the study was to explore patients’ experiences of self-care within MDT. The research questions aimed to identify which factors patients, classified with derangement, perceive as enablers and barriers to following self-care advice given by physiotherapists credentialed in MDT.

Method The study was conducted as a qualitative interview study using semi-structured interviews, which were then analyzed through manifest content analysis.

Results Three categories emerged during the analysis. In the category Enablers through the physiotherapist’s competence and approach, respondents reported a strong therapeutic alliance where the physiotherapist’s assessment and explanation contributed to building trust. In the category Enablers through the patient’s knowledge and participation, respondents stated that they acquired competence and experienced a sense of involvement in the development of their self-care plan. Furthermore, simplicity and quick results contributed to their ability to manage the current episode as well as recurrences. In the category The patient's experience of barriers to adherence to self-care, contextual factors, lack of understanding, a high perceived exercise dose, and insufficient memory support were described.

Conclusion The study identifies perceived enablers and barriers related to adherence to selfcare according to MDT. Although the findings are based on a small number of participants, the study may serve as an initial contribution toward improving adherence when self-care advice is given to patients in an MDT context.
