Abstract [sv]

Bakgrund: Palliativ vård vid demenssjukdom ska lindra lidande och främja livskvalitet, men brister ofta i struktur och kommunikation. Tidiga samtal om palliativ vård är viktiga, men genomförs sällan. Specialistsjuksköterskan har en nyckelroll, framför allt i särskilt boende där vårdetik och modellen De 6 S:n kan stödja en värdig, personcentrerad palliativ vård. Syfte: Syftet var att beskriva sjuksköterskans erfarenheter av att föra samtal om palliativ vård med äldre personer med demenssjukdom på särskilt boende. Metod: Nio sjuksköterskor som arbetade på särskilt boende för personer med demenssjukdom intervjuades om sin erfarenhet av samtal om palliativ vård. Intervjuerna ägde rum i sju olika kommuner i mellersta och södra Sverige. Intervjuerna transkriberades och analyserades med kvalitativ innehållsanalys. Resultat: Resultatet visar att sjuksköterskan möter höga krav, både vad gäller personliga egenskaper och förmågan att skapa rätt förutsättningar för att möjliggöra meningsfulla samtal om palliativ vård med personer med demenssjukdom. Tidiga samtal anses som lämpligt tillvägagångssätt då förmågan att uttrycka sig försvåras allt eftersom demenssjukdomen progredierar. Slutsats: Samtal om palliativ vård med personer med demenssjukdom anses som meningsfulla men utmanande. Sjuksköterskans professionella erfarenhet, relationsskapande och anpassningsförmåga är centrala, men kommunikationssvårigheter och brist på stöd försvårar samtalet. För att möjliggöra samtalen krävs utbildning och tydligare riktlinjer.

Abstract [en]

Background: Palliative care that is given to persons with dementia aims to alleviate suffering and enhance quality of life but often lacks sufficient structure and effective communication. Early conversations about palliative care are important but are infrequently conducted. Specialist nurses hold a key role, especially in residential care facilities, where ethical considerations and the 6 S’s model can support a dignified, person-centred palliative care. Aim: The aim was to describe the nurse's experiences of having conversations about palliative care with older adults with dementia in residential care facilities. Method: Nine nurses working in nursing homes for people with dementia were interviewed regarding their experiences of palliative care dialogue. The interviews were conducted across seven municipalities in central and southern Sweden. All interviews were transcribed verbatim and analyzed using qualitative content analysis. Results: The results indicate that nurses face high demands both in terms of personal qualities and the ability to create appropriate conditions for meaningful conversations about palliative care with persons with dementia. Early conversations are regarded as a suitable approach since the ability to communicate becomes increasingly impaired as dementia progresses. Conclusions: Dialogue about palliative care with persons living with dementia is perceived as meaningful yet challenging. Nurses’ professional experience, relational skills, and adaptability are essential, whereas difficulties in communication and lack of organizational support become barriers for these dialogues. To facilitate such dialogues, enhanced training and clearly defined guidelines are required.
