The art of giving birth : A metasynthesis about positive childbirth experiences from women´s perspectives (English)
Abstract [sv]
Bakgrund: Enligt WHOs riktlinjer är en positiv förlossningsupplevelse ett viktigt mål för alla kvinnor som genomgår en förlossning. En förlossningsupplevelse är unik och en negativ upplevelse påverkas ofta av höga förväntningar eller trauman. Det framkommer i tidigare forskning att barnmorskan är ett viktigt stöd för kvinnan men merparten av tidigare studier fokuserar på negativa förlossningsupplevelser vilket gör att barnmorskans arbete till stor del syftar till att förebygga dessa hellre än att aktivt sträva efter positiva.
Syfte: Syftet är att beskriva kvinnors positiva förlossningsupplevelser.
Metod: Kvalitativ metasyntes med metaetnografisk- och tematisk analysmetod. Datainsamlingen genomfördes i databaserna PubMed och CINAHL med efterföljande kvalitetsgranskning som genomfördes enligt Joanna Briggs granskningsmall. Detta resulterade i totalt 17 vetenskapliga artiklar.
Resultat: Tre huvudteman med totalt åtta underteman identifierades. Huvudtema ett: Att växla från passagerarsätet till förarsätet - kvinnans inre makt som innefattar tre underteman: Motivation som drivkraft, Att ta tag i rodret och ta sig över vågorna samt Föda på egna villkor. Huvudtema två: Trygghet är nyckeln till positiv förlossningsupplevelse som innefattar tre underteman: Barnmorskan - en mantel vävd av kunskap, vänlighet och stöd, Min partner är min sköld samt Upplevelsen speglas av meningsfulla relationer. Huvudtema tre: Att vara i en förlossningsbubbla som innefattar två underteman: En sfär som utstrålar hemmets trygga vrå och Naturens konst - att följa kroppen.
Slutsats: Kvinnors positiva förlossningsupplevelser påverkas av både inre och yttre faktorer. Kontroll och egenmakt är en gemensam och betydelsefull del av upplevelsen. Stödet av barnmorskan är för många kvinnor avgörande vilket ställer höga krav på professionen och därmed är den ömsesidiga relationen mellan kvinnan och barnmorskan extra viktig. Förlossningsmiljön har en betydande roll och bör utstråla lugn och trygghet för att kvinnor ska kunna uppnå en positiv förlossningsupplevelse.
Klinisk tillämpbarhet: Resultatet kan bidra till en ökad förståelse av vad kvinnor upplever som positivt under sin förlossning, kunskapen om detta skulle kunna användas som ett verktyg för barnmorskor vilket kan gynna ett utvecklande arbete inom förlossningsvården.Genom att förstå vilka faktorer som bidrar till en positiv förlossningsupplevelse kan barnmorskor anpassa sitt arbete och främja en optimal miljö för kvinnor under födandet.
Abstract [en]
Background: According to WHO’s guidelines, a positive birth experience is a significant endpoint for all women undergoing labour. A birth experience is unique and a negative experience is often influenced by high expectations or trauma. Previous research shows that the midwife is an important support for, but most previous studies on negative birthexperiences have focused largely on preventing these rather than actively striving for positive ones.
Aim: The aim is to describe women's positive childbirth experiences.
Methods: Qualitative metasynthesis with metaethnographic and thematic analysis method. Data collection was carried out in the databases PubMed and CINAHL with subsequent quality review carried out according to the Joanna Briggs critical appraisal checklist. This resulted in a total of 17 scientific articles.
Results: Three main themes with a total of eight sub-themes were identified. Main theme one: Switching from the passenger seat to the driver's seat - the woman's inner power, which includes three sub-themes: Motivation as a driving force, Taking the helm and overcoming the waves, and Giving birth on your own terms. Main theme two: Safety is the key to apositive birth experience, which includes three sub-themes: The midwife - a cloak woven of knowledge, kindness and support, My partner is my shield and The experience is a reflection of meaningful relationships. Main theme three: Being in a birth bubble, which includes twosub-themes: A sphere that radiates the safe corner of home, and Nature's art - following the body.
Conclusion: Women's positive birth experiences are influenced by both internal and external factors. Control and empowerment are a common and important part of the experience. The support of the midwife is crucial for many women, which places high demands on the profession and thus the mutual relationship between the woman and the midwife is important. The birth environment has a significant role and should radiate calm and security in order for women to achieve a positive birth experience.
Clinical implications: The results can contribute to and increase the understanding of what women experience as positive during their childbirth, the knowledge of this could be used as a tool for midwives which can benefit a developing work within maternity care. By understanding which factors contribute to a positive birth experience, midwives can adapt their work and promote an optimal environment for women during childbirth.
